Surgitron F.F.P.F-EMC高频手术系统治疗腰椎间盘突出症疗效初探

摘　　要：目的:观察Surgitron F.F.P.F-EMC高频手术系统治疗腰椎间盘突出症的近期疗效及手术前、后影像学改变。方法:30例症状、体征与影像学定位诊断相符合的腰椎间盘突出症患者,C型臂X线机透视,局部浸润麻醉下,经相应突出椎间盘安全三角入路,用Surgitron F.F.P.F-EMC高频手术系统进行突出髓核组织的摘除、髓核射频消融、纤维环成型和间盘冲洗。在术后当天、1周、2周、4周评定临床治疗效果,并进行VAS评分;术后1周和4周行MRI检查,测量突出物最高点与椎管后缘垂直距离的变化。结果:术后当天、优13例、良12例;1周,优16例、良10例;2周,优18例、良9例;4周,优23例、良5例。VAS评分由术前的5.3±1.1逐步降为术后当日为4.7±1.1;1周为3.0±0.7,2周为2.3±0.6;4周为2.0±0.5,与术前比较有明显差异;手术后1周突出物最高点与椎管后缘垂直距离明显增大,由术前的9.3±0.9 mm增至12.7±1.0 mm;术后4周进一步增至15.3±0.8 mm。结论:Surgitron F.F.P.F-EMC高频手术系统是治疗腰椎间盘突出症的一种快速、有效的微创治疗方法。Objective： To observe the short-term efficacy and imaging changes before and after operation of surgitron F.F.P.F-EMC high-frequency surgery system for treatment of lumbar disc herniation.Methods： The clinical symptoms and physical signs of 30 lumbar disc herniation cases selected were in compliance with the imaging diagnosis,and treated with surgitron F.F.P.F-EMC high-frequency surgery system through the entrance via security triangle with local anesthesia,under X-ray monitor supervision,to carry out removal of nucleus pulposus,radiofrequency ablation,molding of fibrous ring and washing of intervertebral disc.Follow up of treatment efficacy and VAS scores were carried out at 1 d,1 w,2 w and 4 w after operation according to the MacNab’s.MRI examination was carried out at 1 w and 4 w to measure the variation of distance between highest point of protrusion and posterior border of spinal canal.Results： According to modified MacNab the outcomes were excellent in 13 cases,good in 12 cases at 1 d after operation,excellent in 16 cases,good in 10 cases at 1 w,excellent in 18 cases,good in 9 cases at 2 w,excellent in 23 cases,good in 5 cases at 4 w.The VAS scores decreased from 5.3 ± 1.1 preoperatively to 4.7 ± 1.1 at 1 d,3.0 ± 0.7 at 1 w,2.3 ± 0.6 at 2 w and 2.0 ± 0.5 at 4 w postoperatively,showing statistically significant.The distance between highest point of protrusion and posterior border of spinal canal increas-ed obviously,from 9.3 ± 0.9 mm preoperatively to 12.7 ± 1.0 mm at 1 w,furthermore to 15.3 ± 0.8 mm at 4 w.Conclusion： Surgitron F.F.P.F-EMC high-frequency surgery system is a safe and effective microinvas-ive treatment method for lumbar disc herniation.
