经皮腔内血管成形术治疗老年人下肢动脉硬化闭塞症近期临床疗效观察

摘　　要：目的观察经皮腔内介入治疗老年人下肢动脉硬化闭塞的临床效果和近期疗效。方法 46例共53条肢体84处病变接受治疗,对术前术后踝肱指数(ABI)进行分析。结果 79处病变行经腔内血管成形术(PTA),34处病变行PTA联合支架植入术(PTAS),本组技术成功率94.1%(79/84)。ABI由术前0.31±0.14上升为术后0.71±0.18,差异有统计学意义(P<0.05)。髂动脉、股腘动脉、膝下动脉术后6、12、24个月通畅率分别为96.7%(30/31)、90.3%(28/31)、76.9%(20/26);82.8%(29/35)、78.8%(26/33)、64.0%(16/25);64.7%(11/17)、56.2%(9/16)、44.4%(4/9)。结论经皮腔内介入治疗下肢动脉闭塞安全有效,近期效果好,可重复操作,能迅速重建老年人肢体血流,改善组织供血。Objective To discuss the clinical effects and recent results of percutaneous transluminal intervention in aged patients with lower limb arteriosclerosis obliterans.Methods A total of 46 patients with 53 limbs and 84 lesions were treated.Statistical analysis was conducted on the preoperative and postoperative ankle brachial index（ABI）.Results 79 lesions of lower extremity atherosclerotic occlusions were treated with percutaneous transluminal angioplasty（PTA） and 34 lesions were treated with PTA and stenting（PTAS）.Total success ratio was 94.1%（79/84）.In comparison between pre-and post-operation,the average ABI showed statistic significance,ascending from 0.31±0.14 to 0.71±0.18（P0.05）.At 6,12,and 24 months after operation,the unobstructed rate of the iliac artery,femoral-popliteal artery and below-knee arteries were respectively 96.7%（30/31）,90.3%（28/31）,76.9%（20/26）;82.8%（29/35）,78.8%（26/33）,64.0%（16/25）;64.7%（11/17）,56.2%（9/16）,44.4%（4/9）.Conclusion Percutaneous interventional treatment of peripheral arterial occlusions is safe,effective,its recent results are goods,the operation could repeat in aged patients,and it quickly rebuilds limb blood stream,and improves tissue blood supply.
