Ala-Gln胃肠外营养治疗中度烧伤患者临床疗效观察

摘　　要：目的了解丙氨酰谷氨酰胺（Ala-Gln）外营养治疗中度烧伤成人患者的临床意义。方法将2010年4月至2011年6月入院的中度烧伤患者随机分为治疗组和对照组（准入条件：均为中Ⅱ°烧伤成人）,治疗组给予5%复方氨基酸500ml＋Ala-Gln20g静滴,1次/d,对照组给予5%复方氨基酸500ml静滴,1次/d。创面均给予保痂治疗,每日清创一次,均给予静脉输注抗生素,2次/d,观察患者血常规、肝功能（ALT）变化转归及创面愈合时间。结果治疗组、对照组血常规、ALT比较差异无统计学意义,治疗组浅Ⅱ°创面愈合时间7~10d,深Ⅱ°创面愈合时间18~24d,对照组浅Ⅱ°创面愈合时间9~16d,深Ⅱ°创面愈合时间19~32d,两组比较差异有统计学意义。结论中度烧伤成人患者给予Ala-Gln静脉输注可显著促进创面愈合,缩短住院时间,节约住院费用,具有显著临床应用价值。Objective To explore the clinical effect of Alanyl-Olutamine （Ala-Gln） supplemented total parenteral nutrition （TPN） in the treatment of moderate burn in adults. Methods 371 adult patients with moderate burn admitted from April 2010 to July 2011 in our hospital were randomly divided into the study group （n= 190） and the control group （n=181）. Patients in the study group received injection of 500 ml 5% compound amino acid and 20 g Ala-Gln once a day, while those in the control group were treated with injection of 500 ml 5% compound amino acid once a day. All burn wounds were cleansed every day, and were treated by eschar reservation as well as intravenous infusion of antibiotics twice a day. Routine blood test, serum ALT and wound healing time were investigated. Results Routine blood test and serum ALT showed no statistically significant different between the study group and the control group. Wound healing time in the study group were 7-10 days for superficial second degree burn wounds and 18-24 days for deep second degree burn wounds, which showed statistically significant difference with those in the control group, 9~16 days and 19-32 days respectively. Conclusion For moderate burn in adults, Ala-Gln supplemented TPN can promote wound healing, shorten length of hospital stay and economize the hospitalization costs, which is of great clinical value.
