摘  要:目的:讨论低剂量CT扫描在胸部穿刺活检术中应用的可行性及局限性。方法:回顾分析了2009年8月至2010年9月在16排CT引导下行胸部穿刺活检术患者共80例,其中常规剂量扫描A组(200mA,重建层厚2.0mm,螺距16mm/rot)和低剂量扫描B组(10mA,重建层厚2.0mm,螺距23mm/rot)的患者各40例,比较两组穿刺CT扫描剂量、图像质量和穿刺结果成功率以及并发症之间的关系。结果:常规剂量扫描组CT吸收剂量加权指数CTDIw和平均剂最长度乘积DLP分别为(23.64±2.41)mGy和(172.37±17.63)mGy·cm,穿刺成功率为90%;低剂量扫描组CT吸收剂量加权指数CTDIw和平均剂量长度乘积DLP分别为(1.22±0.12)mGy和(8.56±0.88)mGy·cm,穿刺成功率为85%。两组的辐射剂量差异有统计学意义(P<0.01),而穿刺成功率差异无统计学意义(P>0.05),图像质量分级均能满足穿刺活检的需要,均未发现严重并发症。结论:采用低剂量CT扫描引导胸部穿刺活检是一种值得推荐的技术,可以大大降低辐射剂量。Purpose:To discuss the feasibility of application and limitations of low - dose CT guided coaxial biopsy of lung lesions.Methods:Eighty patients who had undergone CT guided coaxial core biopsy of lung lesions from August 2009 to September 2010 were retrospectively reviewed.The cases were divided into two groups according to the CT protocols they used:group A(200mA,thickness 2.0 mm,pitch 16 mm/ rot) and group B(10mA,thickness 2.0 mm,pitch 23 mm/ rot).Weighted CT dose index (CTDIw),dose - length product(DLP),image quality and biopsy success rate of each group were compared respectively.Results:CTDIw,DLP and biopsy success rate of group A were 23.64±2.41mGy, 172.37±17.63mGy·cm and 90%,respectively,while those of group B were 1.22±0.12mGy,8.56±0.88mGy·cm and 85%,respectively.Image quality grading of both groups could satisfy the need of biopsy.Both groups had no serious complications.Conclusion:Only the CTDIw and DLP appeared significant different between Group A and Group B.Low - dose CT guided coaxial biopsy of lung lesions is a recommended technique,which could dramatically reduce the radiation dose.
