老年男性高血压心肌肥厚患者血清总睾酮水平与心脏交感神经分布变化的关系

摘　　要：目的探讨老年男性高血压心肌肥厚患者血清总睾酮水平与心脏交感神经的分布变化,以及二者间的变化关系。方法入选老年男性高血压患者共10例,年龄60～90岁,依据心脏超声分为心肌肥厚组和非心肌肥厚组。免疫组化的方法半定量分析酪氨酸羟化酶表达。化学发光免疫分析法检测血清总睾酮和雌二醇水平。采用高压液相法检测心室肌去甲肾上腺素含量。结果高血压心肌肥厚患者较非心肌肥厚患者心肌去甲肾上腺素含量升高(Z=-2.309,P<0.05);心交感神经密度降低(Z=-2.611,P<0.05);血清总睾酮水平降低[(2.9±0.5)比(5.1±0.7)nmol/L,P<0.05]。血清总睾酮水平与心交感神经分布密切相关(均P<0.05),是后者的独立危险因素。结论老年男性高血压心肌肥厚患者存在血清总睾酮水平下降和心交感神经密度减低,血清总睾酮水平下降与心交感神经密度减低密切相关。Objective To study the relationship between serum total testosterone and cardiac sympathetic nerve distribution in elderly hypertensive males with cardiac hypertrophy.Methods A total of 10elderly hypertensive males were randomly selected with ages from 60to 90year old and were divided into two groups with and without cardiac hypertrophy respectively.The expression of tyrosine hydroxylase was examined by immunohistochemistry.Results An increase in norepinephrine level（Z=-2.309,P0.05）and a decrease in tyrosine hydroxylase expression were detected in hypertensive patients with cardiac hypertrophy（Z=-2.611,P0.05）.The serum total testosterone level decreased in hypertensive patients with cardiac hypertrophy compared to those without cardiac hypertrophy[（2.9±0.5）vs（5.1±0.7）nmol/L,P0.05].In addition,the serum total testosterone level was closely related to tyrosine hydroxylase expression in hypertensive patients with cardiac hypertrophy,and was an independent risk factor for cardiac sympathetic innervation reduction.Conclusion In the elderly hypertensive males with cardiac hypertrophy,the reductions in serum total testosterone and cardiac sympathetic innervation were detected and serum total testosterone was closely related to cardiac sympathetic innervation.
