摘　　要：目的 探讨自身免疫性大疱病疱液的诊断价值，了解可溶性白介素 2受体（ sIL 2R）、白介素 12（ IL 12）与自身免疫性大疱病的相关性。方法 寻常型天疱疮（ PV） 9例，大疱性类天疱疮（ BP） 31例，获得性大疱性表皮松解症（ EBA） 4例，线状 IgA大疱病（ LAD） 2例。用直接免疫荧光（ DIF）检测患者皮损中自身抗体的类型及沉积部位，用间接免疫荧光（ IIF）检测血清和疱液中自身抗体的类型和滴度，用酶联免疫吸附试验（ ELISA）检测血清和疱液中 sIL 2R、 IL 12的含量。结果 BP和 EBA疱液中均检测到自身抗体。 BP、 PV活动期血清和疱液中 sIL 2R水平明显升高，且随 BP病情好转而下降。 IL 12在 BP、 PV血清中含量高于对照。 BP患者病情严重度不同的两组间血清中两种细胞因子的平均值无差异。结论 疱液具有一定的诊断价值。 sIL 2R可作为监测 BP病情活动性的参考指标。Objective To investigate the diagnostic value of autoantibodies in the serum and blister fluid and to study the expression of sIL 2R and IL 12 in various autoimmune bullous dermatoses. Methods 9 cases of pemphigus vulgaris(PV)， 31 cases of bullous pemphigoid(BP)， 4 cases of epidermolysis bullosa acquisita(EBA) and 2 cases of linear IgA bullous dermatosis(LAD) were included in our study. DIF was carried out to detect the type and site of autoantibody deposition in skin lesions， IIF was carried out to detect the type and titer of autoantibodies in the serum and blister fluid, and ELISA was used to detect the level of sIL 2R and IL 12. Results Autoantibodies were detected in the blister fluid of BP and EBA.The levels of sIL 2R in the serum and blister fluid of BP and PV were higher than those of the controls,and that of BP decreased with the remission of the disease. The levels of IL 12 in BP and PV serum were higher than those of the control. There was no significant difference in the serum levels of sIL 2R and IL 12 between 2 BP groups with different severity. Conclusions The detection of blister fluid has diagnostic value. sIL 2R could act as an indicator of BP activity. The relation between IL 12 and the pathogenesis of BP and PV remains to be studied.
