电针对活动性类风湿关节炎患者外周血与关节滑液中TNF-α与VEGF的影响

摘　　要：目的:观察电针对活动性类风湿关节炎(RA)患者外周血和关节滑液中肿瘤坏死因子α(TNF-α)与血管内皮生长因子(VEGF)的指标改变,确定电针对RA的临床治疗作用。方法:采用随机对照的研究方法,将68例活动性类风湿关节炎患者分为电针组(34例)与对照组(即单纯针刺组,34例)。采取整体与局部相结合的配穴方法,整体取穴以阳经为主,局部取穴依以痛为腧原则,疏经通络,祛风止痛。隔日1次,10次为1个疗程,共治疗3个疗程。结果:电针与单纯针刺对患者外周血和关节滑液中TNF-α与VEGF均有较为显著的作用(P<0.05),电针组对关节滑液中TNF-α的改善作用等同于单纯针刺组,但在外周血的作用上优于单纯针刺组(P<0.05);治疗后在外周血和关节滑液中VEGF降低,电针组的效应均优于单纯针刺组(P<0.05)。结论:电针能有效的降低活动性RA患者外周血和关节滑液中TNF-α与VEGF的含量,改善RA发生、发展的内环境,提高临床效果。Objective： To observe the index change of TNF-α and VEGF in peripheral blood and synovial joint of RA patients by the treatment of electro-acupuncture in order to ensure the clinical cure effect.Methods： 68 cases were randomly divided into an electro-acupuncture group with 34 cases and an acupuncture group with 34cases.And selected acupoints combined global and local and global chose main with yang meridian and local took pain as shu principally,dispersing meridian and dredging collateral,dispelling wind and expelling painful.One time every other day and ten times as a period of treatment and treated for 3 periods.Results： There was an obviously effect of both electro-acupuncture and acupuncture on TNF-α and VEGF in peripheral blood and synovial joint（P0.05）,but seen from the comparison of absolute value and improve value of index change after treatment,electro-acupuncture group had an equal effect with acupuncture group in reduction of TNF-α in peripheral blood and synovial joint,but electro-acupuncture group was better in absolute value of index improvement after treatment（P0.05） and results were fit to clinical cure effects.Conclusion： Electro-acupuncture could effectively reduce the contention of TNF-α and VEGF in peripheral blood and synovial joints,improved the endo-environment of RA and increased clinical effect.
