血清胃蛋白酶原检测在胃部疾病诊断中的意义

摘　　要：目的研究血清胃蛋白酶原(PG)Ⅰ/PGⅡ比值的变化与不同胃黏膜疾病的关系,为胃癌的早期诊断提供筛查依据。方法采用化学发光微粒子免疫分析法测定145例不同胃黏膜疾病患者的血清PGⅠ、PGⅡ含量,并计算PGⅠ/PGⅡ比值,对其进行统计学分析。结果与慢性非萎缩性胃炎组相比,胃溃疡和慢性萎缩性胃炎患者PGⅠ/PGⅡ比值有所下降(P<0.05),胃癌患者PGⅠ/PGⅡ比值明显下降(P<0.001);萎缩型胃炎、胃癌及胃溃疡3组之间PGⅠ/PGⅡ比值差异均无统计学意义(P均>0.05)。结论血清PGⅠ/PGⅡ比值的变化与胃黏膜病变密切相关,对癌前病变及胃癌的早期筛查具有重要的临床意义。Objective To study the relationship between serum pepsinogen（PG） Ⅰ/PGⅡ ratio with various gastric diseases,and provide the reference for screening in the early diagnosis of gastric cancer.Methods Chemiluminesent microparticle immunoassay was used to determine the levels of PGⅠ and PGⅡ and the ratio of PGⅠ/PGⅡ in 145 patients with various gastric diseases.The results were analyzed statistically.Results Compared with patients with chronic non-atrophic gastritis,the serum PGⅠ/PGⅡ ratios in patients with gastric ulcer and atrophic gastritis were lower（P〈0.05）,and that in patients with gastric cancer was significantly lower（P〈0.001）.There was no statistical significance of PGⅠ/PGⅡ ratio among atrophic gastritis,gastric cancer and gastric ulcer（P〉0.05）.Conclusions The changes of serum PGⅠ/PGⅡ ratio are related with gastric pathological changes,and the PGⅠ/PGⅡ ratio has a clinical significance for the early screening of gastric cancer and gastric precancerosis.
