芒硝热敷对心脏起搏器囊袋积血的影响

摘　　要：目的:观察芒硝热敷对心脏起搏器囊袋积血的影响。方法:将符合病例入选标准的23例患者按就诊的先后顺序随机分为常规治疗组(11例)、芒硝组(12例),常规治疗组采用沙袋压迫、大针头穿刺抽血等治疗。芒硝组在常规治疗的基础上加用中药芒硝外敷处理,药量视囊袋积血大小而定,30～60 g/d,将芒硝平铺在1～2层纱布上,并紧贴在积血部位皮肤上,再用热水袋加热(温度60℃左右),3～4次/d,20～30 min/次。治疗期间观察患者囊袋血肿消退情况。结果:常规治疗组和芒硝组所有患者囊袋血肿全部消退,均未发生不良反应、感染、过敏等。常规治疗组血肿消退时间为5.5～7.4天,芒硝组血肿消退时间为1.8～6.3天,2组相比差异具有统计学意义(P<0.05)。血肿最大径线≤5 cm者消退时间最短,其他依次为5～9 cm、≥9 cm。两两比较均有统计学意义(P<0.05)。结论:采用芒硝外敷辅助治疗囊袋积血,疗效满意。Objective： To explore the efficacy of Mangxiao in treating pocket blood accumulation of cardiac pacemaker by fomentation. Method： 23 cases of patients matching the standard were randomized into routine treatment group （11 cases） and MangXiao group （12 cases） in a visiting sequence. Routine treatment group were administered with sandbag constriction,puncture and drawing blood by big needle, etc. MangXiao group were given MangXiao by fomentation combined with routine treatment administered to routine treatment group. The dose of MangXiao depended on the amount of pocket blood accumulation, 30-60 g/d. Spread MangXiao even on the 1-2 layers of gauze; stick the gauze on the skin area of hematomas tightly; apply hot-water bag （with a temperature of 60 degree around）to MangXiao gauze, 3-4 times/d, 20-30 minutes/time.Hematomas regression conditions of the patients were observed during the treatment. Result： All of pocket hematomas disappeared in both of the groups without side effects, infections or allergy, etc. Hematomas regression time of MangXiao group was 1.8-6.3days, shorter than 5.5-7.4 days of routine treatment group with a significant difference （P 〈0.05）. Hematomas regression time of the maximum meridian 〈 5 cm was the shortest, the others were 5-9 cm and 〉 9 cm, there were significant differences among the groups （P〈0.05）. Conclusion： MangXiao in treating pocket blood accumulation by fomentation can achieve satisfying effects as an adjuvant therapy.
