摘　　要：目的探讨全髋置换对治疗髋臼骨折内固定术后失败患者的近期疗效和手术成功要素。方法对12例髋臼骨折内固定失败的患者行全髋置换。4例骨不连患者取出原有内固定,5例节段性缺损者进行结构性植骨,3例髋臼存在巨大的结构性骨缺损,同侧髂骨植骨。均用生物型固定臼杯,股骨侧均采用非骨水泥型假体。结果随访13～48个月,患者无切口感染、假体松动、脱位及坐骨神经损伤及骨溶解征象。骨不连和骨缺损植骨患者均完全愈合。患者髋关节功能均获得明显改善。结论全髋置换用于髋臼骨折内固定失败患者可以取得满意的临床效果。Objective To investigate the short-term efficacy and successful factor of total hip arthroplasty failedinternal fixation of acetabular fractures failed.Methods All the 12 patients were performed total hip arthroplasty.4 cases with acetabular nonunion were performed revision fixation.5 cases with segmental bone defects were reconstructed with autologous femoral head;3 cases with huge defects were reconstructed with autologous iliac bone.All cases were implanted with biological acetabular cup;all femoral sides were cementless prosthesis.Results The follow up time was 13-48 months,no wound infection,implant loosening,dislocation and sciatic nerve injury and signs of osteolysis were observed in the patients.Cases with nonunion and cases with segmental bone defects got union,and all patients got satisfactory results.Conclusion Total hip arthroplasty is a good salvage method for failed internal fixation of acetabular fractures.
