先天性甲状腺功能减低症患儿的生长发育及血清IGFBP-3水平分析

摘　　要：目的分析先天性甲状腺功能减低症(CH)患儿生长发育与胰岛素样生长因子结合蛋白3(IGFBP-3)水平。方法测量42例CH患儿(A组)和41例正常儿童(B组)身高、体重等指标,评价生长发育情况;采用放射免疫分析法测定血清IGFBP-3含量。结果两组儿童出生体重、爬行月龄、独走月龄、萌牙月龄、体重、身高、体重指数(BMI)均无统计学差异。在3-7岁儿童中,A组IGFBP-3明显低于B组[(1894.42±518.12)ng/ml vs.(2444.95±899.44)ng/ml](P<0.01)。3-7岁儿童的IGFBP-3水平与体重、身高呈正相关。结论血清IGFBP-3水平与儿童身高、体重有关。经过早期诊治的CH儿童,生长发育可达正常水平,但其IGFBP-3仍可能偏低。Objective To analyze the status of growth and development and serum insulin-like growth factor binding protein-3（IGFBP-3） in children with congenital hypothyroidism（CH）.MethodsThe growth and development were assessed by criteria including weight and height in 42 CH children（group A） and 41 normal children（group B）.Serum IGFBP-3 was determined with immunoradiometric assay.Results There were no significant differences in birth weight,the first time of climbing and walking,age of tooth eruption,weight,height and body mass index（BMI） between two groups.Serum level of IGFBP-3 in 3-7 years children was lower in group A than that in group B [（1894.42±518.12） ng/ml vs.（2444.95±899.44） ng/ml]（P〈0.01）.The serum concentration of IGFBP-3 was positively correlated to the weight and height in 3-7 years old children.ConclusionIGFBP-3 concentration is associated with children′s weight and height.Growth and development of CH children could be normal when diagnosed and treated in the early stage,but the serum level of IGFBP-3 may be still lower.
