摘 要:目的探讨婴儿期脑部影像学正常的早产低体质量儿脑神经发育状况。方法对103例婴儿期脑部影像学正常的早产低体质量儿进行脑神经发育状况早期随访(6、12、18个月)。结果 103例早产低体质量儿6个月随访时临床观察、格塞尔发展量表(GDS)测评、颅脑B超异常率为26.2%、20.4%和7.8%;12个月随访时临床观察、GDS测评和颅脑CT异常率为14.6%、12.6%和8.7%;18个月随访时临床观察、GDS测评和颅脑CT异常率为9.7%、9.7%和7.8%。18个月时低体质量组临床观察异常率高于高体质量组,差异有统计学意义(P=0.031)。结论部分婴儿期未发现脑部影像学异常的早产低体质量儿脑神经发育存在异常,重者发展成脑瘫或精神发育迟滞,需进行随访观察,以便早发现、早诊断和早康复。Objective To investigate the growth of cranial nerve of underweight premature babies with normal imaging results.Methods Early follow-up(6 months,12 months,and 18 months) was carried out among 103 underweight premature babies with normal imaging results to investigate the growth of cranial nerve.Results The abnormality rates of clinical observation,Gesell Development scale(GDS) evaluation,and ultrasound imaging were 26.2%,20.4%,and 7.8% at 6-month-old,14.6%,12.6%,and 8.7% at 12-month-old,and 9.7%,9.7%,and 7.8% at 18-month-old.The abnormality rates in the underweight group were significantly higher than that in higher-weight group at the 18-month-old(P=0.031).Conclusion Some underweight premature babies with normal imaging may experience abnormal cranial nerve growth,which can develop into cerebral palsy or mental retardation in some severer cases.They should be closely followed up to ensure early detection and prompt treatment.
