临床药师对1例高血压患者开展患者教育和药学监护的探讨

摘　　要：目的:通过参与高血压患者的诊疗,探讨开展患者教育和用药监护的方法。方法:临床药师对1例合并5种疾病、口服药物较复杂的老年高血压患者,根据患者病情给予联合用药监护、服药时间指导以及利用药学服务单对高血压患者进行有针对性的患者教育。结果:向临床提出合理化用药建议,有针对性地进行患者教育,提高了患者用药依从性和治疗效果。结论:有针对性的患者教育,是临床药师有效的工作模式。Objective：To explore the way for patient education and pharmaceutical care through participating in the diagnosis and treatment of a hypertension patient by clinical pharmacists. Methods：According to the illness of the patient suffering from five diseases and complicated medication, pharmaceutical care was given by clinical pharmacists such as guiding the patient to use medicines correctly and the specific education was given to the patient using the pharmaceutical service list. Results：The patient’s compliance with medication and therapeutic effect was improved due to the reasonable proposal for rational drug use and specific education. Conclusion：It was proved to be an effective way to provide specific education on targeted patient by clinical pharmacists.
