2009—2010年我院术后镇痛药应用分析

摘　　要：目的:了解我院术后镇痛注射液的应用现状,为临床合理用药提供参考。方法:采用用药频度(DDDs)、限定日费用(DDC)、DDDs排序、金额排序以及排序比(B/A),分析2009—2010年术后镇痛注射液使用情况。结果:瑞芬太尼金额年均排序居第1位;2010年瑞芬太尼和芬太尼DDDs分别居第1位和第2位;氟比洛芬酯(凯纷)DDC最高,为196元,而哌替啶最低,只有2.2元,吗啡、芬太尼、哌替啶、瑞芬太尼的排序比(B/A)超过1,表明其社会效益和经济效益同步性良好。结论:芬太尼和瑞芬太尼已成为我院术后镇痛的主要注射剂;氟比洛芬酯、帕瑞昔布、丙帕它莫作为非甾体抗炎镇痛药(NASAID)可单独用于术后镇痛,或联合应用阿片类镇痛药可加强镇痛效果、减少阿片类镇痛药用量,从而减轻不良反应。OBJECTIVE： To probe into the status quo of the postoperative use of injectable analgesics in our hospital to provide references for clinical rational use of injectable analgesics. METHODS ： The utilization of injectable analgesics from 2009 to 2010 was investigated in respect of the ordering in DDDs, DDC, DDDs and consumption sum and the sequential ratio （B/A）. RESULTS： Remifentanil dominated the first place in terms of annual average consumption sum over the 2 years. In terms of DDDs, fentanyl took the lead and remifentanil the second in 2010. The DDC of flurbiprofen axetil was the highest at 196 whereas that of pethidine was the lowest at 2.2. The sequential ratios （B/A） of remifentanil, fentanyl, morphine and pethidine were greater than 1, indicating a good synchronism between their economic benefits and social benefits. CONCLUSION： Fentanyl and remifentanil have become the dominant postoperative analgesics in Renmin Hospital of Wuhan University. As NSA1Ds, flurb propacetamol can be used either alone for postoperative analgesia or in combination analgesia effect meanwhile reducing the dosage of opioid drug accordingly attenuating th iprofen axetil, parecoxib and with opioid drug to enhance e adverse reaction
