摘　　要：目的:调查我院各科室手术患者围术期抗菌药物应用情况,分析其用药合理性,以提高围术期抗菌药物合理应用水平。方法:回顾性随机抽查2006年1月—2011年4月全院各科室的218份手术病例资料,了解围术期抗菌药物的使用情况,从适应证、药物选择、各类切口围术期用药时间及持续时间、联合用药等,根据《抗菌药物临床应用指导原则》,对各类切口围术期抗菌药物应用情况进行分析。结果:218例病例围术期使用抗菌药物216例,使用率为99.0%,2例未使用药物,占0.92%。216例围术期患者抗菌药物的应用存在较多不合理现象,主要表现在药物选择不当、药物选择起点高、手术前多日无指征应用抗菌药物、给药时机不当、频繁更换药品、术后用药时间过长等。结论:我院围术期抗菌药物应用不合理现象较多,应加强管理,以保证临床用药安全、有效、经济。OBJECTIVE：To investigate the perioperative antibiotic use for patients undergoing surgery and analyze its rationality so as to improve the rationality in perioperative antibiotic use. METHODS： A total of 218 patients undergoing surgery sampled randomly from clinical departments from January 2006 to April 2011 retrospectively analyzed with regard to the perioperative antibiotic use including the indications and drug selection, timing and duration of antibiotic use for patients undergoing different type of incisions and drug combination etc with the ＂Guiding principles for clinical application of antibacterials＂ used as reference. RESULTS： Of the total 218 cases, 216 （99.0%） used antibacterials and 2（0. 92% ） didn＇t during perioperative period, The irrational drug use in the 216 cases manifested itself variously including improper drug selection, high starting point in drug selection, antibiotic use in the absence of indications, improper medication timing, frequent change in drug types and prolonged antibiotic use after operation. CONCLUSION： The irrational perioperative antibiotic use manifested itself variously in our hospital, thus management should be strengthened to ensure safe, effective and economic antibiotic use.
