斜仰卧位联合改良截石位在微创经皮肾镜碎石取石术中的应用

摘　　要：目的探讨斜仰卧位联合改良截石位在微创经皮肾镜碎石取石术中的应用效果。方法将60例经皮肾镜手术患者分为两组各30例,观察组采用斜仰卧位联合改良截石位行微创经皮肾镜碎石取石术,对照组采用先截石位逆行插管再改斜仰卧位行微创经皮肾镜碎石取石术。比较两组患者在手术中的不适反应及体位摆放所需时间。结果观察组患者与对照组相比,术中头晕、恶心、呕吐不适症状明显减少,观察组体位安置所需时间较对照组短,差异均有统计学意义(P<0.05)。结论在微创经皮肾镜碎石取石术中采用斜仰卧位联合改良截石位减少患者不适反应,缩短体位摆放耗时,对确保手术安全和工作效率效果显著。Objective To discuss the application of inclined supine combined with improved lithotomy position in minimally invasive percutaneous nephrolithotomy（MPCNL）.Methods A total of 60 patients undergoing MPCNL were randomly divided into 2 groups： experiment group（n=30） in which inclined supine combined with lithotomy position in MPCNL was applied,control group（n=30） applying retrograde inserting catheter in lithotomy position before inclined supine was applied in MPCNL.In the operation,discomfort reaction and required posture setting time of patients in two groups were compared.Results Patients in experiment group suffered less discomfort and were with shorter required time for posture setting,which indicated significantly statistic difference（P0.05）.Conclusion In MPCNL,inclined supine combined with lithotomy position can make patient more comfortable,minimize complications and shorten posture-changing time and improve operation safety.
