前列地尔对稳定性心绞痛患者介入治疗术中无复流的影响

摘　　要：目的探讨前列地尔对稳定性心绞痛患者PCI中无复流患者心功能的影响。方法将患稳定性心绞痛、且行PCI术中出现TIMI血流<3级及术中ST段抬高或下移的患者共44例,分为治疗组25例和对照组19例,治疗组(前列地尔脂微球载体制剂10μg);对照组(硝酸甘油200μg)。2组在给药前后记录心电图,比较相关导联ST段的变化;观察术后1个月内心血管事件的发生率。结果治疗组ST段回落≥50%为92.5%,对照组为70.0%;治疗组临床有效率为70.0%,对照组为40.0%(P<0.05)。治疗组心血管事件发生率较对照组降低(P<0.05)。结论前列地尔可改善PCI术中无复流患者左心室功能,减少术后1个月内心血管事件的发生率。objective To investigate the influence of liposomal prostaglandin EI（Lipo-PGE1） on the function of left ventricle and the incidence of cardiovascular events in patients with the no-reflow phenomenon during percutaneous coronary intervention（PCI） of stable angina. Methods 44 pa- tients,who were diagnosed as no-reflow phenomenon during primary PCI for stable angina,were divided randomly into the study group（25 cases） and the control group（19 cases）. In the study group, the patients received intracoronary or introvenous injection of 10 μg Lipo-PGE1;intracoro- nary injection of 200μg nitroglycerin in the control group. The changes in ST segment-related leads of ECG were measured before and after the administration of medications. The cardiovascular events occurred in the 1 months of follow-up period after PCI were assessed. Results Com- pared with the control group, the reduction of ST segment elevation in electrocardiogram≥50% and ST segment returned to baseline completely in study group were more than that in control group（P〈0.05）. The cardiovascular events, including recurrent angina and reinfarction, were less in study group than that in control group（P〈0.05）. Conclusion Lipo-PGE1 has positive effect on the left ventricular function and lowers the incidence of cardiovascular events in patients with the no-reflow phenomenon after PCI for AMI over 1 months.
