摘  要:目的探讨老年患者革兰阴性杆菌肺炎的病原菌构成比及耐药性,为临床医师选择使用抗菌药物提供参考依据。方法对277例老年革兰阴性杆菌肺炎患者的痰及下呼吸道分泌物进行常规细菌培养,用K-B法进行药物敏感试验并统计分析。结果铜绿假单胞菌(PAE)分离率居首位,达26.7%;鲍氏不动杆菌(ABA)第2位,占23.5%;分离率居前5位的病原菌均显示了严重的耐药性,其中PAE、ABA泛耐药株检出率分别为18.9%、21.5%,嗜麦芽寡养单胞菌对亚胺培南100.0%耐药。结论医院务必加强合理使用抗菌药物的管理工作,遏制细菌耐药性快速增长的不良趋势。OBJECTIVE To approach the distribution and antimicrobial resistance of gram-negative bacilli pneumonia to provide the reference for clinical use of antibiotics.METHODS The sputum and lower respiration secretion of 277 patients with Gram-negative bacillus pneumonia were cultured to detect pathogens by routine methods.The susceptibility testing was performed by K-B method.And the susceptibility testing results were analyzed.RESULTS The isolating rate of Pseudomonas aeruginosa(PAE) ranked the top one,reaching 26.7%.Then the following was Acinetobacter baumannii(ABA),accounting for 23.5%.The susceptibility testing results indicated that the serious resistance occurred in the pathogenic bacteria ranked top five.The detection rates of pan-resistant PAE and ABA strains were 18.9% and 21.5%,respectively.Stenotrophomonas maltophilia was with 100.0% resistance rate to imipenem.CONCLUSION Hospital should strengthen the management of antibiotic use to restrain the rapidly increasing tendency of antimicrobial resistance.
