摘  要:目的探讨双水平无创正压通气联合纤支镜肺段冲洗在严重肺挫伤合并急性肺不张患者中的临床应用价值。方法回顾性分析45例严重肺挫伤合并急性肺不张患者的临床资料,比较患者在双水平无创正压通气联合纤支镜肺段冲洗前后的心率、呼吸频率和动脉血气分析指标、胸部影像学表现等方面的差异。结果双水平无创正压通气联合纤支镜肺段冲洗后心率、呼吸频率和动脉血气分析指标、胸部影像学表现均较治疗前明显好转(P<0.05)。结论在常规治疗基础上加用双水平无创正压通气联合纤支镜肺段冲洗是治疗严重肺挫伤合并急性肺不张的有效方法。Objective To summarize the clinical application value about Bi - level noninvasive positive pressure venti- lation joint fiber bronchoseopy lung section flush in serious pulmonary contusion complicated by acute atelectasis. Methods Re- view 45 cases with severe pulmonary contusion complicated acute atelectasis, compared with patients in the Bi - level noninvasive positive pressure ventilation joint fiber bronehoseopy lung section flush before and after flushing of heart rate, respiratory frequen- cy and arterial blood gas analysis index, chest imaging findings of differences. Results After Bi - level noninvasive positive pressure ventilation joint fiber bronchoseopy lung section flush, the heart rate, respiratory frequency and arterial blood gas analy- sis index, chest imaging findings improved significantly (P 〈 0. 05 ) than before treatment. Conclusion Bi - level noninvasive positive pressure ventilation joint fiber bronchoseopy lung section flush based on routine treatment is an effective method to treat serious pulmonary contusion complicated by acuteatelectasis.
