甲氨蝶呤联合宫外孕Ⅱ号方与米非司酮治疗异位妊娠的随机对照试验

摘　　要：目的对比中药宫外孕Ⅱ号方+甲氨蝶呤+米非司酮与米非司酮+甲氨蝶呤保守治疗异位妊娠的疗效。方法将深圳市沙井人民医院2009年1月至2011年1月期间住院行保守治疗的异位妊娠患者95例随机分为试验组45例和对照组50例,试验组给予宫外孕Ⅱ号方+米非司酮+甲氨蝶呤治疗,对照组给予米非司酮+甲氨蝶呤治疗,比较两组的疗效。统计分析采用SPSS 13.0软件。结果治疗后试验组血清β-HCG恢复至正常时间、包块吸收时间及输卵管复通率分别为16.13±8.13天、30.46±7.56天和80%,而对照组分别为22.05±7.15天、39.99±18.26天和75%,两组差异均有统计学意义;而两组在有效率[95.56%(43/45)vs.94%(47/50)]和药物副反应发生率方面,差异无统计学意义。结论中西药结合治疗异位妊娠在协同西药杀胚,缩短血清β-HCG恢复至正常时间和包块吸收时间,以及改善输卵管功能方面均有明显作用。Objective To analyze the e ectiveness of conservative medical treatments for ectopic pregnancy（EP）： methotrexate（MTX） ＋ mifepristone ＋ Ectopic Pregnancy II decoction（EP-II） vs.methotrexate ＋ mifepristone.Meth-ods A total of 95 patients with EP in Shenzhen Shajing Affiliated Hospital of Guangzhou Medical University from January 2009 to January 2011 were randomly divided into two groups： 45 patients in the experimental group were treated with MTX,mifepristone and EP II decoction,while the other 50 patients in the control group were treated with MTX and mifepristone.e e ectiveness of the two groups was analyzed with SPSS 13.0 so ware.Results ere were signi cant di erences in the time of serum β-HCG return to normal（16.13±8.13 ds vs.22.05±7.15 ds,P0.05）,time of EP mass ab-sorption（30.46±7.56 ds vs.39.99±18.26 ds,P0.05） and tubal patency rate（80% vs.75%,P0.05） between the two groups.But there were no signi cant di erences in e ective rate（95.56%,43/45 vs.94%,47/50,χ2=0.0809,P0.05） and side e ects.Conclusion e combination of methotrexate,mifepristone and EP II decoction for ectopic pregnancy is more e ective than mifepristone and methotrexate in coordinately killing the embryo,shortening the time of serum β-HCG return to normal and the time of EP mass absorption,and improving the function of oviducts.
