抗SmD1抗体在系统性红斑狼疮中的临床应用价值

摘　　要：目的探讨抗SmD1抗体在系统性红斑狼疮(SLE)中的临床应用价值。方法将160例确诊为SLE的患者和80例其他风湿病患者及健康人(对照组)先采用酶联免疫吸附试验(ELISA)检测抗SmD1抗体、抗Sm抗体,选取抗SmD1抗体阳性的SLE患者60例,统计分析抗SmD1抗体与患者临床表现、临床活动性的关系。结果 160例SLE患者抗SmD1抗体阳性率为53.1%,抗Sm抗体阳性率为12.5%,80例对照组抗SmD1抗体、抗Sm抗体阳性率分别为5.0%、0。抗SmD1抗体定量与SLE病情活动积分呈正相关(r=0.53,P<0.01)。结论 ELISA定量法检测抗SmD1抗体的敏感性和特异性均较高,可作为诊断SLE的参考指标。抗SmD1抗体定量对判断SLE病情活动有一定的价值。Objective To study the value of clinical applications of anti-SmD1 antibody in systemic lupus erythematosus（SLE）.Methods The anti-SmD1 antibody and anti-Sm antibody in 160 cases of SLE and 80 control subjects were detected by ELISA for quantitative determination.Sixty SLE cases with positive anti-SmD1 antibody were selected and the relations of anti-SmD1 antibody,clinical manifestations and disease activity in these patients were analyzed.Results Among the 160 cases of SLE,the positive rates of anti-SmD1 antibody,anti-Sm antibody were 53.1% and 12.5% respectively.The positive rates of anti-SmD1 antibody and anti-Sm antibody in 80 control subjects were 5.0% and 0% respectively.The level of anti-SmD1 antibody was positively correlated with Systemic Lupus Erythematosus Activity Index（SLEDAI）（r=0.535,P0.01）.Conclusion ELISA has high sensitivity and specificity for the detection of anti-SmD1 antibody.The anti-SmD1 antibody can be used as the reference index in the diagnosis of SLE.The level of serum anti-SmD1 antibody is associated with the activity of SLE.
