导丝法尿管置入在男性尿道重度狭窄中的应用

摘　　要：目的探讨导丝引导下置入尿管治疗男性尿道重度狭窄的实用性与优越性。方法对临床常规或膀胱镜引导置尿管失败的78例尿道重度狭窄患者,在X线透视下以超滑导丝引导置入导尿管。结果 75例均顺利置入尿管;2例因尿道损伤严重、尿道狭窄段形成活瓣或盲腔,1例因残留腔道极窄、经反复试插导丝不能通过,而置管失败。结论对男性尿道重度狭窄经临床常规插尿管失败者,运用导丝技术置入尿管,可有效解除急性尿潴留,为进一步治疗尿道狭窄预留操作空间。Objective To explore the practicality and superiority of guide wire introduced catheter placement in treatment of male severe urethra stenosis.Methods Seventy-eight severe urethral stenosis patients with failed conventional or cystoscopic catheter inserting were involved,and the catheter was inserted under X-ray by the guide of guide wire.Results Catheter was successfully placed in 75 patients.Catheterization failed in 3 cases,including 2 patients with severe urethral injury causing urethral stricture formation or blind valve chamber,1 patient with very narrow residual cavity not allowing insertion of the guide wire.Conclusion For severe male urethral stenosis that clinical catheter inserting failed to manage,guide wire introduced catheter placement can effectively relieve acute urinary retention,promote the operating space of treatment of reserved urethral stenosis.
