摘  要:目的:探讨非超重患者心周脂肪(PAT)含量与冠心病及代谢危险因素之间的关系。方法:选取因胸痛行冠状动脉CTA检查的非超重患者(BMI<24kg/m2)418例,测定PAT含量、冠脉钙化积分,根据冠状动脉直径狭窄≥50%将患者分为冠心病组和非冠心病组;根据血管受累数目及代谢危险因素的数量将患者分组,比较各组间PAT含量。结果:冠心病组[(195.30±77.04)cm3]较非冠心病组[(165.03±56.66)cm3]PAT含量明显增加(P<0.001)。随血管受累支数及代谢危险因素数目的增加,PAT含量呈上升趋势,组间差异有显著性意义(P<0.05)。结论:MSCT测定PAT含量是提示非超重患者冠心病和代谢风险的方法之一。Objective:To explore the relationship between volume of PAT and coronary artery atherosclerosis in non-overweight patients.Methods:A total of 418 patients with body mass index(BMI) under 24kg/m2 were studied.Cardiac multislice computed tomography was used to measure PAT volume,coronary artery calcium score and Gensini score.The patients were divided into coronary artery disease(CAD) and non-CAD group based on the presence of ≥50% stenosis.The patients were grouped according to the number of vessels involved and the number of metabolic risk factors,then PAT volume was compared among the groups.Results:PAT volume was significantly higher in patients with CAD than that in non-CAD subjects [(195.30±77.04)cm3 vs(165.03±56.66)cm3,P0.0001].As the number of vessels involved and the number of metabolic risk factors increased,the volume of PAT increased correspondingly.There was a significant difference among the groups(P0.05).Multiple logistic regression analysis showed that PAT volume was significantly associated with BMI and waist circumference.Conclusion:MDCT measurement of pericardial adipose tissue volume can be applied as one of the indicators of coronary artery disease and metabolic risk in non-overweight patients.
