摘　　要：目的了解不同空腹血糖(FPG)浓度Ⅱ型糖尿病(T2DM)患者胰岛素、C肽释放试验结果与临床的关系。方法 476例T2DM患者根据空腹血糖浓度(FPG)分成三组:FPG≥7.0mmol/L且<11.1mmol/L为A组(357例);FPG≥11.1mmol/L且<14.0mmol/L为B组(81例);FPG≥14.0mmol/L为C组(38例);FPG<6.0mmol/L,餐后2小时血糖浓度(2hPG)<7.8mmol/L正常糖耐量(NGT)为对照组(104例)。对各组口服葡萄糖耐量试验(OGTT)及胰岛素释放试验(IRT)测定胰岛素和C肽结果进行分析,了解糖尿病患者糖代谢和胰岛β细胞的分泌功能。结果Ⅱ型糖尿病患者空腹胰岛素和c肽高于正常人(P<0.05),餐后1h对照组出现峰值,餐后2h糖尿病组达到高峰。结论不同空腹血糖浓度Ⅱ型糖尿病患者胰岛β细胞功能随FPG升高而降低,而测定胰岛素及C肽可用于临床,帮助糖尿病分型,判断病情严重程度及指导治疗。Objective To understand the different fasting plasma glucose（FPG） concentration of type Ⅱ diabetes mellitus（T2DM） patients with insulin,C-peptide release test results and clinical relationship.Methods 476 cases of T2DM patients according to fasting plasma glucose（FPG） was divided into three groups： FPG≥7.0mmol/L and 11.1mmol/L for the group A（357 cases）;FPG≥11.1mmol/L and 14.0mmol/L for the group B（81 cases）;FPG≥14.0mmol/L for the group C（38 cases）;FPG6.0mmol/L,2-hour postprandial plasma glucose levels（2hPG） 7.8mmol/L with normal glucose tolerance（NGT） as the control group（104 cases）.The group of oral glucose tolerance test（OGTT） and insulin release test（IRT） determination of insulin and C-peptide results were analyzed to understand diabetes and glucose metabolism in pancreatic β-cell secretion.Results T2DM patients fasting insulin and c-peptide was higher than normal（P0.05）,postprandial peak value control group 1h,2h postprandial peak in the diabetic group.Conclusion Different fasting plasma glucose concentrations in patients with type Ⅱ diabetes β-cell function decreases with increasing FPG,while the determination of insulin and C-peptide can be used for clinical help diabetes type,to determine the severity of the disease and guide treatment.
