透析患者股静脉置管意外脱管的原因分析及护理对策

摘　　要：目的探讨透析患者意外脱管发生的原因及护理对策,降低意外脱管的发生率。方法收集我科2009年3月至2011年3月期间发生意外脱管的13例临时性股静脉置管进行总结分析。结果发生股静脉置管意外脱管的时间集中在夜间无人看护或陪伴时,地点多为院外患者家中,原因多为患者年龄较大、自理能力较差、夜间睡觉翻身或穿脱衣裤方式不正确。结论通过规范置管护理流程、加强对患者家属自我导管管理的培训指导、重视心理护理等措施,可使透析患者意外脱管发生率降低。Objective To explore the cause and nursing measures of unplanned extubation in patients with femoral vein catheterization for dialysis, and decrease the unplanned extubation incidence. Methods 13 cases of temporary femoral vein catheterizationwith unplanned extubation in our department from March 2009 to March 2011 were selected and analysed. Results The occurrence time of unplanned extubation focused on night time when no care or companionship, and the occurrence focused on patients＇ home;patients with older age or bad ability of self-care, the wrong way to sleep over at night and to dress and undress were the main reasons for unplanned extubation. Conclusion The nursing measures such as standarding the nursing process of catheter insertion, strengthening the training guidance of catheter self-management and paying attention on patients＇ psychological nursing can decrease the incidence of unplanned extubation.
