改良离断性肾盂成形术治疗肾盂输尿管连接处狭窄的体会

摘　　要：目的探讨改良离断性肾盂成形术对肾盂输尿管连接处狭窄的治疗效果。方法采用改良离断性肾盂成形术,对26例先天性肾盂输尿管连接处狭窄患者彻底切除连接处狭窄。结果随访0.5～3年,24例治愈,狭窄解除,吻合口通畅,肾盂积水消失。2例发生再狭窄,再次行狭窄段切除并吻合后治愈。结论改良离断性肾盂成形术是治疗肾盂输尿管连接处狭窄的有效方法,彻底切除肾盂输尿管连接处狭窄,保证吻合口通畅及局部低压状态,是防止术后再狭窄或梗阻的关键。Objective To discuss the therapeutic effects of improved dismembered pyeloplasty in treatment of ureteropelvic junction stenosis.Methods The improved dismembered pyeloplasty was used to thoroughly remove the stenosis of 26 patients with ureteropelvic junction stenosis.Results In the follow up in 6-36 months,24 cases were healed,and their stenosis was removed.The anastomotic stoma was unobstructed,and the nephrydrosis disappeared.Restenosis occurred in 2 cases that were healed after receiving the improved pyeloplasty in the second time.Conclusion Improved dismembered pyeloplasty is an effective way in treatment of ureteropelvic junction stenosis.It can thoroughly remove the ureteropelvic junction stenosis,and keep the anastomotic stoma unobstructed and local low-pressure,which is the key to the prevention of restenosis or obstruction.
