自拟健步汤外洗治疗寒湿痹阻型膝关节骨性关节炎的疗效观察

摘　　要：目的初步观察自拟健步汤外洗治疗寒湿阻络型膝关节骨性关节炎(KOA)的临床疗效。方法将60例寒湿痹阻型KOA患者随机分为健步汤外洗组(中药外洗组)和扶他林软膏外敷组(对照组)。中药外洗组给予自拟健步汤外洗膝关节,2次/d,30 min/次;对照组外敷扶他林软膏,2次/d,14 d为1个疗程。以WOMAC评分作为评定标准进行评价,观察两组的治疗疗效。结果两组治疗后疼痛、躯体功能、僵硬等评分较治疗前显著减少(P<0.05),治疗1周后两组疼痛、躯体功能、僵硬等评分比较,差异无统计学意义(P>0.05),治疗2周后两组疼痛、躯体功能、僵硬等评分比较,差异有统计学意义(P<0.05)。结论自拟健步汤外洗在治疗寒湿阻络型KOA 1周后与对照组疗效相似,在治疗2周后,疗效优于对照组,自拟健步汤外洗治疗KOA疗效肯定,值得推广和进一步研究其作用机制。Objective To observe the clinical curative effects of self-made Jianbu Decoction for external washing in the treatment of knee osteoarthritis with syndrome of cold-damp obstructing the collaterals（KOA）.Methods 60 patients with knee osteoarthritis of syndrome of cold-damp obstructing the collaterals were randomly divided into the Jianbu Decoction for external washing group（Chinese medicine external washing group） and the Voltaren Ointment for external coating group（control group）.The Chinese medicine external washing group used self-made Jianbu Decoction for the external washing of knee joint twice per day and 30 minutes per time while the control group used Voltaren Ointment for external coating twice per day,with 14 days as one treatment course.With the WOMAC score as the evaluation standards,the curative effects of both groups were observed.Results After treatment,the pain,physical function,stiffness scores and other scores significantly reduced compared with those before treatment（P 0.05）.One week later,the pain,physical function,stiffness scores and other scores between the two groups showed no statistically significant differences（P 0.05）.Two weeks later,the pain,physical function,stiffness scores and other scores between the two groups showed no statistically significant differences（P 0.05）.Conclusion The self-made Jianbu Decoction for external washing has similar curative effects with the control group in the treatment of KOA of syndrome of cold-damp obstructing the collaterals after one week,but has superior curative effects to the control group after two weeks.The self-made Jianbu Decoction for external washing has positive curative effects in the treatment of KOA,thereby worthy of promotion and further research into its mechanism.
