摘 要:目的探讨川芎嗪治疗对胎儿生长受限(FGR)孕妇血液流变性的影响。方法将34例FGR孕妇随机分为川芎嗪组17例(A组)和营养支持组17例(B组),另选择同期在我院定期产检并分娩的正常孕妇30例作为正常妊娠对照组(C组)。A组给予营养支持的同时加用川芎嗪注射液治疗,B组仅予以单纯营养支持治疗,7 d为1个疗程;C组除建议左侧卧位外,不予以其他处理。监测三组孕妇血液流变学参数、脐动脉血流及新生儿出生体重。结果治疗后,A组孕妇血液流变学参数、脐血流阻力明显低于B组,差异有统计学意义(P<0.05);FGR孕妇全血低切(5/s)黏度(LBV)值与脐血流阻力呈正相关,与新生儿出生体重呈负相关。结论川芎嗪能显著改善FGR孕妇血液流变性,降低子宫胎盘循环阻力,改善胎盘绒毛微循环,促进胎儿生长发育,增加新生儿出生体重。Objective To assess the impaction of Tetramethylpyrazine on hemorheology of pregnant woman with fetal growth restriction(FGR).Methods 34 cases of pregnant women with FGR were randomly divided into Tetramethylpyrazine group(group A) and nutritional support group(group B),each group had 17 cases.Besides,30 normal pregnant women who taken regular inspection at the corresponding time were selected as the control group(group C).Group A was treated with Tetramethylpyrazine combined with nutritional support for 7 days,group B was only given the nutritional support therapy,and group C was suggested to keep left-lateral position without any other treatment.Parameters such as maternal hemorheology,umbilical artery blood flow velocity,newborn birth weight were measured and compared among the three groups.Results After treatment the value of hemorheology parameters and umbilical artery blood flow resistance indexes of group A were significantly reduced than those of group B,the differences were statistically significant(P0.05).There was a significant positive correlation between the blood viscosity at low shear rate(LBV) of pregnant women with FGR and umbilical artery blood flow resistance indexes,which has a negative correlation between the birth weight of the infants.Conclusion Tetramethylpyrazine can obviously improve the hemorheology of FGR patients,reduce uteroplacental resistance and take a favorable turn of placental villus microcirculation,which also can promote fetal growth and development,increase newborn birth weight.
