健脾益肾活血中药对早期2型糖尿病肾病的临床疗效分析

摘　　要：目的探讨健脾益肾活血中药对早期糖尿病肾病(DN)的临床疗效。方法研究对象为我院收治的140例早期2型DN患者,将其随机分为洛汀新组、健脾活血组、益肾活血组、健脾益肾活血组、健脾活血洛汀新组、益肾活血洛汀新组及健脾益肾活血洛汀新组,每组各20例,8周为1个疗程,观察各组治疗前后相关指标的变化。结果健脾益肾活血洛汀新组治疗后血纤溶酶原激活物抑制剂-1(PAI-1)、血内皮素-1(ET-1)、24 h尿白蛋白排泄率(UAER)、血纤维蛋白原(Fg)、血超敏C反应蛋白(hsCRP)、血转化生长因子-β1(TGF-β1)、血肿瘤坏死因子(TNF-α)、血糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、空腹血糖(FPG)、餐后2 h血葡萄糖(2 h PG)等指标显著降低,趋于正常水平;血总胆固醇(TC)、血三酰甘油(TG)、血低密度脂蛋白胆固醇(LDL-C)、血高密度脂蛋白胆固醇(HDL-C)显著好转;健脾益肾活血洛汀新组与其他各组间相比,以上15个指标差异均有统计学意义(均P<0.05)。结论健脾益肾活血中药对早期2型DN疗效确切,可延缓病情进展,提高患者的生活质量,值得临床上推广应用。Objective To investigate the clinical effects of Chinese medicine of strengthening spleen and tonifying kidney and invigoration blood in the treatment of early diabetic nephropathy（DN）.Methods 140 patients with early type Ⅱ DN admitted to our hospital were randomly divided into the lotensin group,strengthening spleen and invigoration blood group,tonifying kidney and invigoration blood group,strengthening spleen and tonifying kidney and invigoration blood group,strengthening spleen and invigoration blood Lotensin group,tonifying kidney and invigoration blood Lotensin group,and strengthening spleen and tonifying kidney and invigoration blood Lotensin group,each group had 20 patients,with 8 weeks as a treatment course.The changes of relevant indicators before and after treatment were observed.Results In the strengthening spleen and tonifying kidney and invigoration blood Lotensin group,the PAI-1,ET-1,UAER,Fg,hsCRP,TGF-β1,TNF-α,HbA1c,FINS,FPG,2 h PG and other indicators significantly reduced and tended to the normal level;TC,TG,LDL-C and HDL-C significantly improved;Compared with other groups,the strengthening spleen and tonifying kidney and invigoration blood Lotensin group was statistically different in the above 15 indicators（P0.05）.Conclusion Chinese medicine of strengthening spleen and tonifying kidney and invigoration blood is effective in the treatment of early type Ⅱ DN,can delay disease progression and improve patients＇ quality of life,thereby worthy of clinical application.
