摘 要:目的探讨预处理对CO2气腹所致肝脏损伤的影响。方法 SD大鼠24只,随机分为3组,每组8只:假气腹组(C组)、气腹组(P组)和预处理-气腹组(PP组)。P组闭合法建立CO2气腹,腹压为15mmHg,维持90min后解除气腹,PP组预处理为5min充气和5min放气,反复两次后建立气腹,维持90min后解除气腹。C组不建立气腹,麻醉时间同PP组。解除气腹后60min取血测定血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、超氧化物歧化酶(SOD)、丙二醛(MDA)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平,光镜观察肝组织病理学变化,进行损伤程度评分。结果与C组比较,P组、PP组血清ALT、AST、MDA、TNF-α、IL-6水平显著增加(P<0.05),SOD显著降低(P<0.05);与P组比较,PP组ALT、AST、MDA、TNF-α、IL-6水平显著降低(P<0.05),SOD显著增加(P<0.05);但3组间肝组织病理损伤评分差异无统计学意义(P>0.05)。结论预处理能减轻CO2气腹所致肝脏损伤,机制可能与减轻氧自由基导致的组织脂质过氧化反应、抑制TNF-α、IL-6释放有关。Objective To explore the effects of preconditioning on CO2 pneumoperitoneum - induced liver injury in rats. Method Selecting 24 SD rats and randomly divided into 3 groups ( n = 8 ), they were control group ( group C), pneumoperitoneum group ( group P) and preconditioning - pneumoperitoneum group ( group PP). The rats in group P were subjected to CO2 pneumoperitoneum for 90min with 15mmHg intra - abdominal pressure and then deflation. The rats in group PP were subjected 2 cycles of preconditioning, followed by 90min of pneumoperitoneum and deflation. Preconditioning was defined as 5 rain of pneumopefitoneum followed immediately by 5 min of deflation. Rats in the group C were subjected to a sham operation without pneumoperitoneum and anesthesia time was equal to that of group PP. 60 min after deflation, blood samples were taken for measuring of serum alanine transaminase (ALT) , glutamic - oxal (o) acetic transaminase (AST) , superoxide dismutase ( SOD), malonaldehyde ( MDA), tumor necrosis factor alpha ( TNF-α) and interleukin (IL) - 6 levels. Histopathologic injury scores of liver tissue were also examined. Results The level of ALT, AST, MDA ,TNF-αa nd IL - 6 were significantly higher and the level of SOD was significantly lower in group P and PP than in group C ( P 〈 0.05 ). Compared with group P, the level of ALT, AST, MDA ,TNF-α and IL- 6 in group PP decreased and SOD increased significantly ( P 〈 0. 05 ). But the hepatic injury scores of 3 groups had no significant difference ( P 〉 0.05 ). Conclusions Preconditioning can attenuate pneumoperitoneum - induced hepatic injury by inhibiting TNF-α and IL - 6 release and reducing lipid peroxidation which induced by oxygen radicals.
