坎地沙坦酯对2型糖尿病合并高血压患者血管内皮功能和踝臂指数的影响

摘　　要：目的探讨坎地沙坦酯对2型糖尿病合并高血压患者血管内皮功能和踝臂指数（ABI）的影响。方法46例新诊断的2型糖尿病合并高血压患者随机分为常规治疗组和坎地沙坦酯组，每组23例，测定治疗前及治疗12周后血压、ABI、空腹血糖（FPG）及肱动脉内皮依赖性舒张功能（EDD）。结果常规治疗组治疗前FPG为（8．56±2．96）mmol／L，治疗后为（5．99±0．76）mmol／L（P〈0．05），坎地沙坦酯组治疗前FPG为（8．38±2．06）mmol／L，治疗后为（5．51±0．56）mmol／L（P〈0．05），两组FPG均下降明显。治疗后常规治疗组EDD为（4．78±2．79）％，坎地沙坦酯组为（5．31±3．02）％，两组EDD变化差异有统计学意义（P〈0．05）。治疗后常规治疗组ABI为1．17±0．14，坎地沙坦酯组为1．24±0．16，两组ABI上升差异有统计学意义（P〈0．05）。结论坎地沙坦酯可降低2型糖尿病合并高血压患者的血压及空腹血糖，可以提升其踝臂指数，改善血管内皮功能。Objective To investigate the influence of candesartan cilexetil on vascular endothelial function and ankle-brachial index in patients of type 2 diabetes mellitus with hypertension. Methods Forty-six patients of type 2 diabetes with hypertension were randomly divided into the routine treatment group and candesartan group （each n=23 ）. The blood pressure, ankle-brachial index （ABI）, fasting plasma glucose（FPG） and brachial artery endothelium-dependent dilation （EDD） were detected before and after the treatment. Results The level of FPG of the patients decreased significantly in both groups （P〈0.05）. In the routine treatment group before and after of the treatment, the level of FPG was （8.55 ±2.96）mmol/L and （5.99 ±0.76）retool/L, respectively. In candesartan group before and after of the treatment, the level of FPG was （8.38±2.06） mmol/L and （5.51±0.56）retool/L, respectively. In the routine and candesartan groups the differences of the before and after treatment in ABI were significant （P〈0.05） （1.17±0.14） and （1.24±0.16）, respectively. Conclusion Candesartan can control the level of blood glucose and blood pressure in the patients of type 2 diabetes mellitus with hypertension, increase ABI, and improve vascular endothelial function.
