摘 要:目的:探讨采用心脏专用软件包(Argus)测量活体肝移植术后残肝体积的可行性。方法:45例肝移植供体,术后常规MRI扫描及钆贝葡胺增强后容积内插值体部检查(VIBE)序列扫描,容积数据传入并行后处理工作站行多平面重建(MRP)。首先在viewing模式下逐层绘出残肝外形轮廓,计算残肝体积,并计算时间。然后相同数据导入Argus软件计算残肝体积,并计算所用时间。采用直线相关比较手工测量和Argus测量残肝体积的相关性。采用配对t检验对比2种检查方法所需时间的差异。结果:手工测量和Argus测量残肝体积分别为(1 105.6±228.2)和(1 105.2±225.9)cm3,手工测量和Argus测量残肝体积间有相关关系(r=0.999,F=23 680,P=0.000,P<0.05)。手工和采用Argus测量所需时间分别为(14.73±1.43)和(5.45±0.80)min,差异有统计学意义(t=22.81,P=0.000<0.05)。结论:Argus软件与手工测量相比,对于活体肝移植术后残肝体积测量相关性好,并且省时、简便,具有很高的临床应用价值。Objective To investigate the feasibility of measuring remnant liver volumes with Argus method. Methods Forty- five candidates after living donor liver transplantation underwent liver MRI on a 3.0-T MR unit. Volume interpolated body examination (VIBE) was performed after the administration of gadobenate dimeglumine. The VIBE data was transferred to the diagnostie workstation, and then multi planar reformation (MPR) images were acquired. First, two observers manual drawn the liver shape and calculated remnant liver volumes. Second, the same data was transferred to Argus software, remnant liver volumes was calculated. Each measurement was timed. The correlation between two measurements was analyzed. The time needed for Argus and manual method was compared with paired t test. Results The remnant liver volumes of Argus and manual method was (1105.6±228.2) and (1105.2±225.9) cm3 and the correlation coefficients were 0.999. The time for manual method and Argus method was (14.73±1.43)and (5.45±0.80)min, respectively. There was significant difference between Argus and manual methods(t=22.81, P=0.000, P〈0.05). Conclusion Compared with manual method, usage of the Argus software for remnant liver volumetric measurement is accurate and rapid. Argus has higher clinical value for volumetric measurement in remnant liver volume.
