地佐辛联合酮咯酸氨丁三醇用于全麻病人术后镇痛的临床研究

摘　　要：目的观察地佐辛复合酮咯酸氨丁三醇用于全麻病人术后静脉自控镇痛(PCIA)的可行性,以及镇痛效果和安全性。方法选择全麻手术患者136例,术后行PCIA治疗,随机分为3组。A组:地佐辛15mg,酮咯酸氨丁三醇180mg,雷莫司琼0.6mg;B组:酮咯酸氨丁三醇180mg,舒芬太尼50μg,雷莫司琼0.6mg;C组:舒芬太尼100μg,雷莫司琼0.6mg。三组均用0.9%生理盐水稀释至100mL。术毕接PCIA泵,持续给药量2mL.h-1,自控量0.5mL/15min,并由患者完成对疼痛的评分。观察用药后循环、呼吸的变化,并进行VAS、BCS及Ramsay评分,观测可能出现的不良反应。结果 3组患者通过VAS和BCS评分及镇痛效果不存在显著性差异(P>0.05),都能提供有效的镇痛。A组患者术后恶心、呕吐、头晕及瘙痒发生率明显比B、C组少;呼吸抑制发生率与B相当,但明显低于C组,相比有差异(P<0.05)。结论 3组不同组方药物行PCIA治疗都能提供安全而有效的镇痛效果,A组选用地佐辛和酮咯酸氨丁三醇相配伍行PCIA治疗,其镇痛效果确切,不良反应少,镇静分数低,安全性高,比B、C组更能体现其实用价值。OBJECTIVE To study the analgesic efficacy, feasibility and safety of Dezocine combined with Ketorolac Tromethamine in the PCIA treatment. METHODS Observation on select ASA I - Ⅱ level in patientswith elective surgery under general anesthesia in 136 cases. PCIA after surgical treatment. Randomly dlvldea into 3 groups.Group A： Dezocine 15rag, Ketorolac Tromethamine 180μg, Ramosetron 0.6mg; Group B： Ketorolac Tromethamine 180/μg, Sufentanil 50μg, Ramosetron 0.6mg; Group C： Sufentanil 100μg, Ramosetron 0.6mg.Three groups are diluted with saline to 100mL, background infusion rate was 2mL·h^-1, bolus dose was 0.05mL;lock out time was 15min. Observation after circulation, respiratory changes, and get VAS BCS and Ramasy scores while observing any possible adverse reaction.RESULTS Visual analogue scale （VAS） and Bruggrmann Comfort Scale（BCS） have no differences between three groups（P 〉 0.05）, they all can provide effective analgesia;Nausea, vomit, swirl, pruritus patients were significant increase in group A compared with group B or group C; And respiratory depression patients have no defference between group A and group B, and were significant increase in group A compared with group C （P 〈 0.05）. CONCLUSION Three Prescriptions all provide safe and effective analgesia. But Dezocine combined with Ketorolac Tromethamine in group A, low composed elfeet, High security and generate less side effect.
