急性期疼痛程度及血清前炎性细胞因子水平与老年带状疱疹后遗神经痛的关系

摘　　要：目的通过检测老年带状疱疹（HZ）患者急性期痛觉视觉模拟评分（VAS）和血清前炎性细胞因子水平，探讨HZ急性期疼痛程度和前炎性细胞因子水平与带状疱疹后遗神经痛（PHN）发病的相关性。方法选择HZ急性期患者60例，根据随访结果分为A组（PHN未发病）和B组（PHN发病），同时选择年龄和性别相匹配的健康体检者为对照组。记录HZ急性期疼痛最严重时VAS评分，采用流式模拟微球分析技术检测HZ急性期血清TNF-α、IL-6、IL-1β水平。结果B组急性期疼痛最严重时VAS评分高于A组（P〈0．05）。A、B组TNF—α、IL-6、IL-1β水平均高于对照组（P〈0.05）；与A组比较，B组IL-6水平增高（P〈0.05）；A、B组间TNF-α、IL-1β差异无统计学意义（P〉0．05）。结论老年HZ患者急性期前炎性细胞因子水平较对照组显著增高；HZ急性期疼痛程度及前炎性细胞因子的过度表达可能与PHN发病有关。Objective TO investigate the association of visual analogue scales （VAS） and serum proinflammatory cytokine levels with the development of postherpetic neuralgia （PHN） in elderly patients with acute herpes zoster （HZ）. Methods Sixty aged patients with acute herpes zoster were divided into two groups： group A （PHN not developed） and group B （PHN devel- oped）; 25 healthy subjects matched by age and gender served as controls. The VAS scores were recorded during acute phase, and serum TNF- α, IL-6 and IL-1β levels were measured by cytometric bead array technique. Results There was no significant difference in gender and age between groups. The VAS scores in group B were significantly higher than those in group A （P〈0.05）. Serum TNF- α, IL-6 and IL-1βlevels were increased in groups A and B, compared to control group （P〈0.05）; serum IL-6 level in group B was significantly higher than that in group A （P〈0.05）. Conclusion The severe pain and increased serum levels of IL-6 may be associated with the development of PHN.
