摘　　要：目的 通过对宁波地区二级以上医院围术期临床用血情况分析,为提高科学合理用血提供客观数据.方法 对2007年宁波地区35家二级以上医院住院手术患者用血情况进行调查,统计术中及术后72h手术患者各种血液成分输注量和输血指征符合情况,分析临床用血的合理性.结果 22 316例次输血中,成分输血率为99.6%,8 525次红细胞输注属于无指标输血,即38.2%的围术期用血仅凭临床医生的经验实施.有完整输血指标检测记录的手术患者,依据〈临床输血技术规范〉进行合理性与否的判断,红细胞输注不合理率为48.9%,围术期血浆用量占临床用量的51.2%,不合理率63.7%.自体输血量占围手术患者用血量的10.2%.结论 宁波地区尽管成分输血及自体输血开展良好,但不合理输血现象较严重,急需大力普及临床合理用血知识及制定相关限制性输血干预措施,指导临床医生合理、安全地使用血制品具有重要意义.Objective To survey the utilization of perioperative blood-using in secondary hospitals and above in Ningbo Municipality. Methods The perioperative blood use in 2007 were retrospectively investigated in 35 secondary and tertiary hospitals in Ninbo Municipality. The data of blood composition infusion, transfusion indications, the quantities of blood transfusion during and 72h after surgery were collected analyzed. Results in 22,316 cases of blood transfusion, the blood-constitution transfusion reached to 99.6%; 8525 cases received RBC transfusion （38.2%） did not have transfusion indications and just based on the doctors＇ experience. In surgical patients having complete records of transfusion index, 48.9% was the unreasonable RBC transfusion as judged by Technical specifications of clinical blood transfusion. Perioperative plasma transfusion accounted for 51.2% of clinical blood use, in which 63.7% was unreasonable use. The amount of autologous blood transfusion accounted for 10.2% of perioperative blood transfusion. Conclusion The blood-constitution transfusion and autologous blood transfusion have been developed well in Ningbo area. But the problems of unreasonable blood transfusion are still serious, and necessary intervention measures should be taken to promote the rational and safe blood use perioperatively.
