2型糖尿病伴Sweet综合征1例

摘　　要：本文报道1例53岁女性T2DM伴Sweet综合征患者,因"血糖升高4年,双下肢皮肤皮疹、溃烂1周"入院。既往有高血压病、亚急性甲状腺炎史。查体发现双下肢可见皮疹、溃烂。糖耐量试验确诊T2DM。皮肤活检病理报告示真皮浅层皮肤附件及小血管周围见较多中性粒细胞浸润,血管扩张。胰岛素及糖皮质激素治疗有效。T2DM伴Sweet综合征的发生可能与炎症因子、免疫功能变化有关。A 53-year-old woman complained of hyperglycemia for 4year,skin rash and ulceration of lower extremity for 1week before hospitalization.She had hypertension and subacute thyroiditis,skin rash and ulceration of lower extremity.Oral glucose tolerance test showed a diagnosis of type 2diabetes mellitus.Pathological report on the skin showed that diffuse inflammatory infiltrate of neutrophils in the superficial layer of dermis,surrounding small vessels vasodilatation can be observed in cutaneous lesions.Insulin and glucocorticosteroid therapy were effective.Sweet＇s syndrome may be associated with both inflammatory factors and changes of immunological function in patient with type 2diabetes mellitus.
