针刺治疗缺血性中风后遗症临床研究

摘　　要：目的观察针刺治疗缺血性中风后遗症的临床疗效及针刺效应。方法 60例缺血性中风后遗症患者随机分为两组,对照组30例用常规药物治疗,治疗组30例在常规药物治疗的基础上施以针刺。治疗21 d后,比较两组神经功能缺损评分、Barthel指数;并对治疗组30例患者进行红外热像仪检测。结果两组治疗前后神经功能缺损评分比较差异均有统计学意义(P<0.05),两组治疗后神经功能缺损评分比较差异有统计学意义(P<0.05),治疗组改善程度优于对照组。两组治疗前后Barthel指数评分比较差异均有统计学意义(P<0.05),两组治疗后Barthel指数评分比较差异没有统计学意义(P>0.05)。治疗组患者患侧阳白、地仓、合谷、太冲穴穴位表面温度在治疗前后组内比较差异均有统计学意义(P<0.05),治疗后高于治疗前。结论针药并用治疗在改善缺血性中风后遗症患者的神经功能缺损、日常生活活动能力方面与单纯药物治疗比更有优势。针刺能提高阳白、地仓、合谷、太冲穴的皮肤表面温度。Objective To observe the clinical effect and action mechanism of acupuncture in treating ischemic stroke sequelae（ISS）.Method Sixty patients with ISS were randomized into two groups.The 30 patients in the control group were treated with routine medication,and the other 30 in the treatment group were intervened by acupuncture plus medication.After 21-day treatment,the two groups were compared with each other in terms of neurological deficit score（NDS） and Barthel Index（BI）.The 30 patients in the treatment group received additional test with infrared thermal imager.Result Both groups showed significant difference in comparing NDS after treatment（P0.05）,and the result of NDS in the treatment group was significantly better than that in the control group after treatment（P0.05）.Both groups showed marked difference in comparing BI score after treatment（P0.05）,but the difference of BI score between the two groups after treatment was insignificant（P0.05）.The superficial temperature at acupoints Yangbai（GB 14）,Dicang（ST 4）,Hegu（LI 4）,and Taichong（LR 3） on the affected side became markedly higher after treatment（P0.05）.Conclusion Compared with medication alone,acupuncture plus medication has obvious advantage in improving NDS and daily life activity for patients with ISS.Acupuncture increases the superficial temperature at Yangbai（GB 14）,Dicang（ST 4）,Hegu（LI 4）,and Taichong（LR 3）.
