佩尔地平和压宁定在高血压脑出血急诊开颅术麻醉诱导期降压应用比较

摘　　要：目的 :比较佩尔地平和压宁定在高血压脑出血急诊开颅术麻醉诱导期降压作用。方法 :45例高血压脑出血 ,急诊开颅血肿清除术病例随机分成 2组 :A组 15例 ,诱导前 5min静脉注射佩尔地平 30ug/kg ,观察给药后第 1、 3、 5min ,诱导后第 3min ,插管即刻 ,插管后第 1、3、 5min的收缩压、舒张压、平均动脉压、心率的变化 ;B组 30例 ,同期注射压宁定 0 5mg/kg,观察各项参数。结果 :各组降压效果显著 ,和术前比较P <0 0 5或P <0 0 1,但常规剂量下A组降压效果显著强于B组 ;A组心率和术前比较显著加快 ,B组心率无改变。结论 :对于高血压导致的急性脑出血患者 ,在麻醉诱导期前后 ,佩尔地平和压宁定均能显著地降低血压 ,在常规剂量下前者作用显著强于后者 ;佩尔地平有显著加快心率的作用 。Objective:Comparing the effect of blood pressure reduction perdipine with urapidil during anesthesia induction for patients with acute cerebral hemorrhage due to hypertension. Method:Forty five patients undergoing emergency craniotomy were randomly divided into two groups.The patients(n=15) in group A were given intravenously 30ug/kg of perdipine 5 min before induction of anesthesia,and the patients(n=30) in group B 0 5mg/kg of urapidil at the same time as group A.In both of groups,systolic blood pressure,diastolic blood pressure,mean blood pressure and heart rate were monitored and recorded on the 1 st ,the 2 nd and the 5 th min after injection of the drugs,on the 3 rd min after anesthesia induction,on the time of intubation and on the 1 st , 3 rd and the 5 th min after intubation.Results:After the injection of the drugs,blood pressure was reduced significantly in comparison with those before the injection in both of groups(P<0 01 or P<0 05).However,perdipine with routine dosage has stronger effect on reducing blood pressure and increasing heart rate on any stage after the injection of the drug as compare with urapidil(P<0 01 orP<0 05). Conclusion:Both of perdipine and urapidil have the effects on decreasing blood pressure around anesthesia induction in the patients with cerebral hemorrhage due to hypertension,but perdipine has stronger effect than urapidil.Next,perdipine can increase the heart rate but not urapidil.
