帕利哌酮缓释片和利培酮对首发精神分裂症患者认知功能的影响

摘　　要：目的:探讨帕利哌酮缓释片与利培酮对首发精神分裂症患者认知功能的影响。方法:50例首发精神分裂症患者随机分为帕利哌酮缓释片组(n=25)和利培酮组(n=25),均单药治疗。在治疗前和治疗8周末采用阳性和阴性症状量表(PANSS)、威斯康星分类卡片测验(WCST)、连续作业测验(CPT)、数字划销测验(CT)及修订韦氏成人记忆量表(WMS-R)评价临床疗效及认知功能。结果:治疗后两组PANSS总分及各因子评分均较治疗前显著下降,差异有统计学意义(P<0.01);帕利哌酮缓释片组有效率为80%,利培酮组有效率为76%,两组相比差异无统计学意义。治疗前后帕利哌酮缓释片组WCST,CPT,CT及WMS-R各项指标比较差异有统计学意义(P<0.05,P<0.01),利培酮组仅WCST及WMS-R指标比较差异有统计学意义(P<0.05);治疗8周末两组间各项观察指标比较差异无统计学意义。结论:帕利哌酮缓释片治疗首发精神分裂症疗效和利培酮相当,对于认知功能的改善总体相似,但在执行功能和注意力方面帕利哌酮缓释片略优于利培酮。Objective： To compare the effects of paliperidone extended-release（ER） tablets and risperidone on cognitive function in patients with first-episode schizophrenia.Methods： Totally 50 patients with first-episode schizophrenia were randomly enrolled and divided into 2 groups（n=25 in each group）.They were treated with the monotherapy of paliperidone ER or risperidone for 8 weeks.Positive and Negative Syndrome Scale（PANSS）,Wisconsin Card Sorting Test（WCST）,Continuous Performance Task（CPT）,Number Cancellation Test（CT） and Wechsler Memory Scale-Revised（WMS-R） were used to assess the clinical efficacy and cognitive function.Results： At the end of study,the total score and sub-scores of PANSS were significantly reduced in both groups（P0.01）.The effective rates in two groups（80% and 76%） had no statistically significant difference.Before and after treatment there were statistically significant differences in indexes of WCST,CPT,CT and WMS-R in paliperidone ER group（P0.05 or P0.01）,but only in indexes of WCST and WMS-R had statistically significant differences in risperidone group（P0.05）.There were no significant differences in all observational indicators between two groups at the 8th week of treatment.Conclusion： Paliperidone ER and risperidone have similar clinical efficacy and beneficial effects on the cognitive function for treating first-episode schizophrenia;however,paliperidone ER is slightly superior to risperidone in improving attention and executive function.
