不同疗法治疗普通型手足口病837例临床疗效观察

摘　　要：目的比较中医、西医及中西医结合疗法对手足口病普通型的临床疗效。方法收集全国12个中心837例手足口病普通型患者,采用临床队列研究方法,分为3组,中医组(409例)治以清热解毒、化湿透邪为原则的中药颗粒剂组方,西医组(124例)予对症及抗病毒治疗,中西医组(304例)两种疗法联合应用,疗程7天。观察3组患者退热起效时间、体温恢复正常时间、疱疹消退时间、咽痛减轻时间及消失时间、临床非特异性症状消失时间,并比较3组临床疗效。结果中医组、中西医组、西医组临床总有效率分别为99.8%、100%、100%,3组临床疗效比较差异无统计学意义(P>0.05)。各组发热患者的退热起效时间比较差异无统计学意义(P>0.05);发热患者的体温恢复正常时间中医组与西医组均早于中西医结合组(P<0.05或P<0.01)。疱疹消退时间中医组早于西医组和中西医组(P<0.01);西医组早于中西医组(P<0.01)。中医组患者咽部疼痛减轻时间和消失时间均早于中西医组(P<0.01)。临床非特异性症状和体征完全消失时间中西医组早于中医组与西医组(P<0.01);中医组早于西医组(P<0.01)。结论 3不同疗法对手足口病普通型病例均有效,其中中医疗法效果最佳。Objective To evaluate the clinical effect of traditio nal Chinese medica l（TCM）,western medical（WM） and integrated traditional and western medical（IT WM） therapies for popular type of hand foot and mouth disease（HFMD）.Methods To tally 837 infants with popular type of HFMD from 12 centers across China were in volved in the research.Clinical cohort study was performed and the patients wer e divided into the TCM group,WM group and ITWM group.TCM granules for clearing heat,resolving toxin,eliminating dampness and out-thrusting evils were admin istered to 409 patients in the TCM group.The symptomatic and antiviral therapy w as administered to 124 patients in the WM group.The combined therapy was admini stered to 304 patients in the ITWM group.The course of treatment lasted for 7 d ays.The time of pyretolysis,body temperature recovery,herpes regress,pharyng algia alleviation and extinction,non-specificity symptoms relief in the three g roups was observed and compared.Results The total effe ctive rate of the TCM gro up,WM group and ITWM group was 99.8%,100% and 100% respectively,the differenc e between was no statistically significant（P0.05）.The pyretolysis time betwe en groups was no significantly different（P0.05）.The time of body temper ature recovery in the TCM group and WM group was significantly shorter than that in t he ITWM group（P0.05 or P0.01）.The time of herpes regress in the T CM group w as significantly shorter than that in the WM group and ITWM group（P0.01）.The time of pharyngalgia alleviation and extinction in the TCM group was significant ly earlier than that in the ITWM group（P0.01）.The time of non-specifici ty sym ptoms relief in the ITWM group was significantly earlier than that in the TCM gr oup and WM group（P0.01）,but it was significantly earlier in the TCM grou p tha n that in the WM group（P0.01）.Conclusion Three therapies are all effective for HFMD,especially TCM therapy.
