中医外治三联法治疗膝骨性关节炎60例临床观察

摘　　要：目的评价中医外治三联法治疗膝骨性关节炎的临床疗效和安全性。方法将120例膝骨性关节炎患者随机分为治疗组和对照组,每组60例,治疗组给予压腿锻炼、手法弹拨和中药熏敷三联法治疗,对照组口服抗骨增生胶囊治疗。两组分别于治疗前及治疗后7、14、21天对患膝进行评分,并对评分结果进行统计分析。结果两组患者临床疗效比较,治疗组60例中临床控制0,显效13例,有效33例,无效14例,总有效率为76.7%。对照组60例分别为0例、1例、20例、39例及35.0%,两组比较差异有统计学意义(P<0.05),治疗组优于对照组。症状包括:①夜间休息时疼痛、②晨僵或起床后疼痛、③行走时疼痛、④最大行走距离、⑤关节功能等。两组患者治疗前及治疗后不同时间单项症状疗效比较,每组各单项症状治疗前后(治疗前分别与治疗后第7天、14天、21天)评分比较差异均有统计学意义(P<0.05),且治疗组治疗后(第7天为症状①、②、③;第14天为症状①、③、④;第21天为症状①、②、③、④)评分优于对照组(P<0.05)。除治疗组中1例患者出现皮肤过敏反应终止治疗外,其余患者未出现不良反应。结论中医外治三联法治疗膝骨性关节炎能有效缓解疼痛,改善关节功能,具有较高安全性。Objective To evaluate the curative effect and safety of the triplex ext ernal treatment of TCM for knee osteoarthritis.Methods Totally 120 patients wer e randomized into treatment group and control group with sixty in each.The trea tment group accepted triplex external treatment of TCM including lower-limb exercise,manipulation and external use of Chinese medicine,while the control group accepted Kanggu Zengsheng Capsule（Capsule for treating hyperostosis）.Both group s were scored at the 7th,14th and 21st day after the treatment.Then the score results were statistically analyzed.Results The clinic ally-controlled,excellen t,effective and ineffective cases were 0,13,33 and 14 in the treatment group,while 0,1,20 and 39 respectively in the control group.The total effective ra te of the treatment group and control group was 76.7% and 35% respectively,the difference between was statistically significant（P0.05）.The single symp tom s uch as rest pain at night,early morning stiffness,pain after getting up,walki ng pain,max walking distance and joint function was analyzed before and after t reatment.There was significant difference in symptom scores at 7th,14th and 21 st day after the treatment in both groups（P0.05）.The scores of treatmen t gro up were superior to those of the control group with statistically significant（P 0.05）.There was no adverse effect case in both groups except for one case of unexplained skin allergy in the treatment group.Conclusion The Triplex external treatment of TCM is safe and could relieve pain and improve joint function in t reating knee osteoarthritis.
