类风湿关节炎关节功能分级与中医证候的关系

摘　　要：目的从影像学角度探讨类风湿关节炎(RA)关节功能分级与中医证候的关系。方法对150例RA患者进行关节功能分级、X线分期和中医辨证分型,分析X线分期与关节功能分级的关系以及中医证候与关节功能分级的关系。结果 RA关节功能分级与X线分期呈正相关(P<0.01)。湿热痹阻证与肾气虚寒证、肝肾阴虚证比较,差异有统计学意义(P<0.01);寒湿痹阻证与肾气虚寒证、肝肾阴虚证比较,差异亦有统计学意义(P<0.01)。湿热痹阻证与寒湿痹阻证患者关节功能多为I、Ⅱ级;肾气虚寒证与肝肾阴虚证患者关节功能多为Ⅱ、Ⅲ级。结论 RA患者X线分期越高,关节功能分级也越高;湿热痹阻证、寒湿痹阻证关节功能Ⅲ级分布最少,而肾气虚寒证、肝肾阴虚证则以关节功能Ⅲ级分布最多。Objective To approach the joint function degree of rheumatoid a rthritis（RA） and its relationship between TCM syndromes according to the imagin g.Methods Joint function classification,X-ray stagin g and TCM syndromes were p erformed on 150 RA patients.The relationship between the joint function classif ication with X-ray staging and TCM syndromes was analyzed respectively.Results There existed positive correlation between the joint function classification wit h X-ray staging in RA patients（P0.01）.The difference between damp-heat impedi ng syndrome with kidney qi deficiency cold syndrome and liver-kidney yin deficiency s y ndrome was statistically significant（P0.01）.The difference between cold-damp impeding syndrome with kidney qi deficiency cold syndrome and liver-kidney yin deficiency syndrome was statistically significant as well（P0.01）.The joint func tion classification of damp-heat impeding syndrome and cold-damp impeding syndrome wa s almost grade I or gradeⅡ,while kidney qi deficiency cold syndrome and liver-ki dney yin deficiency syndrome was almost grade Ⅱ or grade Ⅲ.Concl usion There is positive correlation between the joint function classifications with X-ray stagin g.Grade Ⅲ distributed widely in the kidney qi deficiency cold syndrome and liver-k idney yin deficiency syndrome,while narrowly in damp-heat impeding syndrome and c old-damp impeding syndrome.
