摘 要:目的提出一项评估心肌灌注的新指标:桡动脉心内膜下心肌活力率(SEVR),并将从桡动脉压力波形直接获取的SEVR与颈动脉SEVR进行相关性分析验证其有效性。方法在200例20~74岁的健康人(男性99人,女性101人)中,将表面张力传感器分别测量颈动脉SEVR和桡动脉SEVR10。结果尽管颈动脉SEVR比桡动脉SEVR更高,但是桡动脉SEVR与颈动脉SEVR仍然高度相关(r=0.81,P<0.001)。男性的桡动脉SEVR明显比女性更高。多元线性逐步回归分析显示:心率、年龄、脉压差、收缩压是桡动脉SEVR的独立影响因素。结论直接从桡动脉压力波形获取的SEVR可以提供与颈动脉SEVR相似的信息,桡动脉SEVR可能替代颈动脉SEVR作为评估心血管系统疾病的一项主要指标。Objective To propose a new index for assessment of myocardial perfusion: the radial artery subendocardial viability ratio(SEVR),and to examine whether SEVR obtained directly from radial artery pressure waveforms(radial SEVR) can provide information comparable with carotid arterial SEVR measurements.Methods In a cross-sectional study of 200 apparently healthy subjects(99 men and 101 women) aged 20-74 years,the carotid SEVR and radial SEVR were measured using applanation tonometry.Results Radial SEVR was strongly correlated with carotid SEVR(r=0.81,P〈0.001),although radial SEVR was much higher than carotid SEVR.Male subjects had a slightly higher radial SEVR than females.Stepwise multiple-regression analysis showed that heart rate was a potent predictor of radial SEVR in addition to age,pulse pressure and systolic BP.Conclusion These results suggest that SEVR obtained directly from radial arterial pressure waveforms can provide equivalent information to carotid arterial SEVR,and has the potential as a surrogate marker of cardiovascular diseases.
