主动脉壁内血肿的MSCT诊断

摘　　要：目的探讨多层螺旋CT（MSCT）诊断主动脉壁内血肿的价值。方法收集主动脉壁血肿12例．对其MSCT表现进行同顺性分析。结果12例按Stanford分刷，A型3例，B型9例。MSCT表现：主动脉壁呈新月形增厚12例．厚度为10-16mm，增强后未见强化；内膜钙化斑块内移7例；穿透性溃疡征3例；内膜渗漏4例；合并胸腔积液5例，心包积液1例．左髂总动脉及髂外动脉夹层l例；3例治疗后随访可见主动脉壁内血肿厚度呈动态变化。结论MSCT能清晰显示主动脉壁内血肿的部位、累及范围、厚度及有无并发症，埘临床诊治有重要的指导意义。Objective To investigate the wdue of MSCT in diagnosing the aortic intramural hematoma. Methods MSCT scanning was performed in 12 cases with aortic inlramurai hematoma,the imaging findings were retrospectively analyzed. Results Three pa tients were diagnosed as Stanford type A,and the others as type B. The MSCT finding showed as follows：crescent-shaped thickening { 10-16 ram） of the aortic wall without enhancement at contrast-enhanced CT scan（12 cases） ;intimal displacement of calcium（7 ca ses） ;penetrating ulcer（3 cases） ;intimal leakages（4 cases） ;pleural effusion（5 cases） ,pericardial effusion（ 1 case） , left common lilac artery and external iliac artery dissection（1 case） ; dynamic change of hematomas during the follow-up period（ 3/3）.Conclusion MSCT can clearly show the locations,extent,thickness and complications of aortic intramural hematoma,which has important value for clinical diagnosis and treatment.
