Castleman病CT及MRI表现特征与病理类型

摘　　要：目的探讨Casdeman病的CT及MRI影像学表现及诊断价值。方法回顾性分析8例经手术后病理征实Castleman病的Cq、及MRI征象。结果局限型5例，弥漫型3例．病理分型透明血管型7例，浆细胞型1例；共检出72个肿大淋巴结，所自病灶均边缘清晰、锐利，71个病灶密度均匀，1个病灶内钙化。所柯病灶均显著强化，其中3个病灶内局部强化不均。MRI检合2例．病灶T1WI均星均匀中等信号，T2Wl呈高信号，1例信号均匀，l例信号混杂。透明血管型较浆细胞型强化型为移著。术前CT、MRI叫确诊断5例（67．5％），误诊2例，诊断不明1例。结论Castleman病具有特征性CT及MRI表现。Objective To investigate the CT and MRI features of Castleman disease and diagnostic value of CT and MR imaging. Methods CT and MRI findings of 8 cases with castleman disease confirmed by pathology were retrospectively analyzed. Results Of 8 cases, 5 cases were classified as localized Castleman disease and 3 cases were classified as disseminated castleman disease. 7 eases were classified as hyaline-vascular type and 1 case was classified as plasma cell type by pathology. On CT images, 72 lesions found in all 8 patients were manifested as well-circumscribed soft tissue masses. Among these, 71 lesions were homogeneous density, 1 le sion had calcification. All lesions were enhanced markedly, 3 lesions among of which showed inhomogeneous enhancement followed corn rast-enhanced scans. The degree of enhancement of hyaline-vascular type was stronger than that of plasma-cell type. For 2 cases of MRI examination, the lesions were homogeneous medial signal on T1 Wl, 1 lesion was homogeneous high signal and 1 lesion was inhomogeneous on T2WI. Before operalion, 5 cases were diagnosed definitely, 2 cases were misdiagnosed, and 1 case diagnosis was unknown by CT and MRI. Conclusion CT and MRI findings of castleman disease are characteristic and of high diagnostic value.
