摘  要:目的探讨超声弹性成像与声触诊在甲状腺良恶性结节中的诊断价值。方法选择甲状腺结节患者125例,每位患者选取1个实性或以实为主的典型结节,术前均进行常规超声(CU)、超声弹性成像(UE)、声触诊组织量化(VTQ)检查,对每个病灶进行弹性成像分级和声触诊组织量化速度分组,诊断结果与术后病理对照。结果甲状腺结节良恶性组间超声弹性分级差异及声触诊速度值分组检出率差异均具有统计学意义(P<0.05)。但良、恶性结节组UEⅢ级检出率差异无统计学意义。UE与VTQ诊断准确率分别与UE+VTQ+CU三者联合诊断比较,差异有显著性(P<0.05)。CU诊断准确率与UE、VTQ比较,差异均无明显统计意义(P>0.05)。UE+VTQ+CU三者联合诊断准确率与CU比较,有一定程度提高,但差异无明显统计意义(P>0.05)。结论超声弹性成像、声触诊和常规超声三者联合应用,有助于提高甲状腺良恶性结节诊断的准确性。Objective To explore diagnostic value of ultrasonic elastography (UE)and virtual touch tissue quanti- fication(VTQ) in benign and malignant thyroid nodules. Methods Conventional ultrasonography(CU), UE and VTQ were used to examine 125 thyroid nodules of 125 patients before operation. UE grades and VTQ velocity classification were done for every nodule. Their diagnostic results were contrasted with pathology after operation. Results Therewere significant differences in detectable rate of both UE grades and VTQ velocity groups between benign and malig- nant nodules groups(P〈0.05). But the detectable rate of UE Ⅲgrade showed no significant difference between benign and malignant groups(P〉0.05). The diagnostic accuracy of both UE and VTQ were significantly lower than that of UE+VTQ+CU(P〈0.05). There was no significant difference in diagnostic accuracy among UE, VTQ and CU (P〈 0.05). The diagnostic accuracy of UE+VTQ+CU was slightly high compared with those of CU, but there was no significant difference(P〉0.05). Conclusions The combined application of UE,VTQ and CU can promote the diag- nostic accuracy of benign and malignant thyroid nodules.
