硝酸异山梨酯及环磷腺苷葡胺对慢性心功能不全患者血浆N末端脑钠肽水平的影响

摘　　要：目的探讨硝酸异山梨酯及环磷腺苷葡胺对慢性心功能不全患者血浆N末端脑钠肽(NT-proBNP)水平的影响。方法将我科2009年5月至2010年11月64例慢性心功能不全住院患者,按入院时间先后分成硝酸异山梨酯组(32例)和环磷腺苷葡胺组(32例),在常规治疗基础上,2组分别予静脉泵入硝酸异山梨酯10～20 mg/d及环磷腺苷葡胺180 mg/d,并检测患者治疗前及治疗1周后血浆NT-proBNP水平。结果 2组治疗1周后NT-proBNP水平较治疗前均显著下降。硝酸异山梨酯组治疗前(10.02±5.48)μg/L,治疗后(5.41±4.98)μg/L;环磷腺甘葡胺组治疗前(8.80±5.38)μg/L,治疗后(5.54±5.13)μg/L。2组治疗前比较及治疗1周后比较NT-proBNP水平差异均无统计学意义(P>0.05)。结论常规治疗联合硝酸异山梨酯或环磷腺苷葡胺治疗均能明显降低慢性心功能不全患者血浆NT-proBNP水平,且两药物疗效差异无统计学意义(P>0.05)。Objective To investigate the effects of isosorbide dinitrate and meglumine adenosine cyclophosphate on the level of N-terminal pro-brain natriuretic peptide（NT-proBNP） in blood plasma of patients with chronic cardiac insufficiency.Methods A total of 64 patients with chronic cardiac insufficiency,who were hospitalized from May 2009 to November 2010 were divided into 2 groups according to the time order of their admission,i.e.the isosorbide dinitrate group（n=32） and the meglumine adenosine cyclophosphate group（n=32）.In addition to routine treatment,the 2 groups were administered isosorbide dinitrate（10-20 mg/d） and meglumine adenosine cyclophosphate（180 mg/d） respectively with mini-pumps.Then,the levels of NT-proBNP in blood plasma were measured both before treatment and 1 week after treatment.Results One week after treatment,the levels of NT-proBNP in blood plasma for both groups all reduced significantly,when compared with those before treatment（P0.01）.And the levels of NT-proBNP pre and post treatment and also 1 week after treatment were all not statistically significant for the 2 groups（P0.05）.Conclusion Routine treatment coupled with isosorbide dinitrate or meglumine adenosine cyclophosphate could significantly reduce the levels of NT-proBNP in blood plasma in patients with chronic cardiac insufficiency and no statistical differences could be seen in the therapeutic effect of the two drugs（P0.05）.
