摘　　要：目的了解四川省泸州市海洛因成瘾者社区美沙酮维持治疗脱失和保持的影响因素。方法采用定性研究的方法,于2008年11月至2009年8月,在泸州市美沙酮维持治疗(MMT)门诊选取20名已经脱失和30名依从性较好的服药人员,进行深入访谈,归纳总结服药人员脱失和保持情况的影响因素。结果泸州市参加社区美沙酮维持治疗的服药人员脱失的主要原因是外出打工,其次是失去经济来源、违法犯罪被抓和偷吸毒品。保持的主要原因是源于良好的药物疗效、家人关爱和较好的社会支持,门诊医生的关爱引导也是重要影响因素之一。结论美沙酮良好的药物疗效、家人关爱、社会关心与支持等,是海洛因成瘾者能够保持社区美沙酮维持治疗的重要因素。把MMT门诊和社区戒毒、社区康复工作有机结合,以降低脱失率。Objective To explore the factors associated with retention or drop out of methadone maintenance treatment （MMT） among herorin users in Luzhou county, Sichuan province. Methods According to the program design, from Dec. 2008 to Aug. 2009, twenty drug users and thirty residents who have good compliance were observed in a MMT community. The in-depth or open-ended interview were used to collect information about factors that related to retention or drop out of MMT. Results Among drug users, go out to work for others were the main reasons of drop-out, The others include： no income, use heroin stealthily and crime during treatment; factors con- tributed to retention included： Effective treatment, and better family or social support. In addition, positive and ac- tive intervention of the medical workers was also an important factor for maintaining higher rate. ConclusionGood efficacy of methadone,family care,social care and support are important factors which MMT among heroin users. The organic integration of drug and AIDS prevention, maintenance treatment of drugs and community drug treatment, community rehabilitation combine to reduce dropout rate.
