宫颈解脲支原体感染与不良妊娠结局关系的Meta分析

摘　　要：目的 探讨孕妇宫颈解脲支原体（UU）感染与不良妊娠结局的关系.方法 采用计算机检索1991～2011年中国期刊全文数据库、中文科技期刊全文数据库（VIP-维普）、万方-数字化期刊全文库,并辅以文献追溯的方法,收集国内公开发表的有关宫颈UU感染与不良妊娠结局的相关文献.应用Rev Man 4.2.10软件进行Meta分析.结果 将11个研究结果 加权合并,累计宫颈UU阳性者1 952例,阴性者3 280例.Meta分析结果 显示胎膜早破[OR=6.64,95%CI（4.56～9.67）]、早产[OR=3.23,95%CI（2.62～3.98）]、胎儿窘迫[OR=1.53,95%CI（1.09～1.81）]、新生儿窒息[OR=1.48,95% CI（1.14～1.92）]、新生儿肺炎[OR=3.84,95% CI（2.78～5.30）]、低出生体重儿[OR=2.58,95% CI（2.01～3.31）]、剖宫产[OR=1.47,95%CI（1.11～1.94）]、产褥病率[OR=2.86,95%CI（2.23～3.68）]的发生在宫颈UU阳性组和阴性组中差异有统计学意义.结论 目前国内孕妇宫颈UU感染与不良妊娠结局发生有显著关联.Objective To discuss the relationship between cervical ureaplasma urealyticum （UU） infection and adverse pregnancy outcomes. Methods After a recursive literature searching for the published reports between 1991 and 2011 in CNKI database, VIP database and Wanfang database, literatures about the relationship between cervical UU infection and adverse pregnancy outcomes were collected. The Cochrane Collaboration＇ s software Rev Man 4.2.10 was used to analyze the data. Results Results of 11 studies were weightedly merged. A total of 1 952 cervical UU positive cases and 3 280 cervical UU negative controls were collected. The differences in the incidence of premature rupture of membranes, premature delivery, fetal distress, neonatal asphyxia, neonatal pneumonia, low birth weight infant, cesarean section and puerperal morbidity between UU positive group and negative group were significant with 0R（95% CI） at 6.64 （4.56-9.67）, 3.23 （2.62-3.98）, 1.53 （1.09-1.81）, 1.48 （1.14-1.92）, 3.84 （2.78-5.30）, 2.58 （2.01-3.31）, 1.47 （1.11- 1.94） and 2.86 （2.23-3.68） respectively. Conclusion Cervical UU infection in pregnant women at present is obviously associated with the incidence of adverse pregnancy outcomes.
