摘  要:目的:研究新生儿缺氧缺血性脑病(HIE)临床分度与CT征象的相关性。方法:对54例HIE的临床及CT影像资料进行回顾性分析。对所有患儿均进行临床分度、CT分度,并按是否合并颅内出血分组。结果:临床分度与CT分度呈中度相关,临床分度随CT分度的增加而增加;与CT值呈中度相关,CT值随临床分度的增加而降低;是否合并颅内出血在临床分度上有统计学差异,合并颅内出血以中至重度为多,两组在CT值上无明显差异。结论:临床与CT分度在中至重度的判定上有良好的一致性,但在轻度的判定有一定的出入;随着脑的缺氧缺血性损害的加重CT值明显下降,颅内出血的几率也增加;对新生儿HIE最重要的诊断依据仍然是病史和临床表现,CT扫描作为辅助手段对判断病情的轻重则有重要价值。Objective: To research the correlation between clinical degree and CT features of the neonates with hypoxic - ischemic encephalopathy (HIE) . Methods: The clinical data and CT imaging data of 54 neonates with HIE were analyzed retrospectively. All the neonates underwent clinical grading and CT grading, then they were divided into different groups according to whether they were combined with cerebral bleeding. Results: There was a moderate correlation between clinical grade and CT grade, clinical grade increased with the increase of CT grade, which was correlated with CT value moderately ; CT value decreased with the increase of clinical grade; there was significant difference in the clinical grade between the neonates with cerebral bleeding and the neonates without cerebral bleeding; most of the neo- nates with cerebral bleeding had moderate to severe HIE, there was no significant difference in CT value between the two groups. Conclusion: The consistency of clinical grade and CT grade is good for predicting moderate to severe HIE, but there is a certain difference in predicting mild HIE ; CT value decreases significantly with the aggravation of HIE, the probability of cerebral bleeding also increases; the most important diagnostic criteria for neonatal HIE are still medical history and clinical manifestations, CT scanning can be used as an assisted technique to predict the degree of the disease.
